Jan. 13 (Monday) = Door's locked. Client awake, calling me. Hadn't complain much of pain like the past days when as long as I got in she says back hurts and I would start day with massage, rub, and the hot towel. Says she hadn't had much sleep last night, got little sleep only. And also says she owes Noelia $5.50 for Chinese food last Saturday. Client had waffle and tea before or around 10 am and had took Hydrochlorothiazide after. Put pain cream on client's back. Tidied up room, kitchen and continued dryer. Was telling about a Windmill recipe on the ad, but I can't find the ad, not on kitchen table, not on trash, and not on client's walker as she said. Says a recipe about a chicken and we could get the chicken from Smart and Final. Then asked her what she wants for lunch, and says we could set aside the chicken recipe from the ad and I could go to Akins for lunch. Put cream on back and massaged. Had asked client what time she took pain med, says she hadn't take any. Then says she wasn't sure if she had taken a pain med., maybe morning. Client wrote check to be cashed in to bank, $30. I called DZ Akins and put in usual order, talked to the Mexican man I've also talked to for order often. Says I'd pick it up before 11 am. I left passed 11 am for bank, Carlos: teller. Had cashed in the $30. At Akins next and picked up order, felt the bag was cold so I checked on the items. Bread and 1/2 lbs. corned beef was cold eventhou I ordered warm. But only told the guy who handed me the picked up order that I ordered hot 1/2 lbs. corned beef. Lady employee changed it. Paid with client's cash. Dropped by to Ralphs on my way to get dog foods. Paid with cash as well. Got back, and prepared lunch, made corned beef sandwich and zucchini, steamed. Served lunch 11:47 'sh am, the sandwich, steamed zucchini and water for drink. Also returned change,$13.27. Checked on client passed 12:20 'sh pm to give her pain medicine, says she just took it (Ibufrofin) and I gave her 1/2 of the Vicodin, told her it's her other pain medicine. Client in pain. Went on and used pain cream, client asked for hand Body cream too to be tried on her back, which we did. Also used hot towel as she had suggested. Client says she will pay me first for what she owes me for lunches, from BK and Subway. Paid me $12. (Owes me $6.40 and $6.88) but only told her $12., so she paid me $12. At 1 pm, told her I have to go, I have to go to agency's office. Client says she want to hold off the payment for the week, not sure if she have the money in her bank. Put on clean dress on client (she took of her gown) and turned on tv before leaving. Client sleepy when I left. Left passed 1:15 'sh pm.
Jan. 14 (Tuesday) = Can hear dog barks as I got out of the car. Client on her way in the kitchen to take dog out, client sat on kitchen chair and I put dog outside. Says back pain, and client got exhausted form the walk. Asked for muscle rub on her back, which I used. Had asked her if she had taken pain med., says yes, two tablets, taken together, 2 Ibufrofin tablets, about 3 hours ago. Told client she shouldn't have done it, she was told to take 1 every 8 hours. Noticed client coughing. Asked for Alkazeltzer which she had, when she walked into the vacant room and lied down. Sometimes says about back pain, but wasn't complain much like the past days. Shampooed vacant room. Client says she should take shower, so I prepared shower and told her to try, but not force herself to walk in shower. Client did well and got into the shower. Texted her son about client taking 2 Ibufrofin tablet, and her coughing. Client had shower. Phone rang. Tried to answer in the kitchen but cord fall off, and Merrilee walks in, with camera. Continued shower, and came out to see Merrilee as client what she needs, as client wanted. Was told she needs to take my photo as I won caregiver of the month. Then left. Continued client's shower, keeps asking for hotter water, and already on maximum but still wasn't hot enough for her. Assist client on dressing 10:40 'sh am. And she had taken her Hydrochlorothiazide med. client lying in the bed. Got a missed call from her son, about getting client into Scripps Hospital, and concerned about client's coughing, as he remembered that how client's husband's pneumonia sits in. Only left voicemail about client having shower when she called me so hadn't noticed cp. Continued shampooing vacant room's carpet. Client thinking about calling Ralphs for coffee cakes and Vons for jello. Told her I'd change her bedsheets first. Tidied up client's room, swept carpet, and changed bed sheets and almost done when client walks inside. Says back pain, and lied down in the bed. Emptied commode and soaked soiled bedsheets and washed soaked clothes. Client decided Popeyes for lunch, Tuesdays special, leg and thigh. Says she have to pay Noelia for the $5.50 she owes her for Chinese food. Client gathered her coins for lunch, got over $4. Turned on tv before going, and client asking if she could take another Ibufrofin, told her not, as she already took 2 as she said when I came in, says ok, she wouldn't. Left 11:50 'sh am. Got leg and thigh chicken, mild, and decided to add a biscuit. Got back passed 12 noon. Warmed up thigh and biscuit and made steamed spinach. Woke up client for lunch around 12:20 'sh pm, chicken thigh, steamed spinach and the biscuit and water for lunch. Made baked potato to add to the chicken leg I had set aside for client's meal later. Called Rite-Aid pharmacist and told her about getting in at 9, and being told by client she took Ibufrofin, 2 tablets of 600 mg. And asked if it's ok for client to have another one again as she client wanted to take another one before I go to get lunch. Pharmacist says two 600 mg, taken together would be 1200 and that could cause liver damage. Ad client should increase hours of taking med again, like wait 8 to 10 hours, don't take any ibufrofin pretty soon, maybe late tonight or tomorrow but vicodin can be taken. So walk into client's room, was sound asleep, woke her up to give 1/2 of vicodin and I was told she took another Ibufrofin after I left to get lunch. Called Rite-Aid pharmacist again, this time talked to a male pharmacist and told him story again, and added that I just went to clients room to give her 1/2 of vicodin and client told me she took another Ibufrofin when I went to get lunch. Asked if client could still have the vicodin, and was told client could go have the vicodin as it wasn't acetapmenophine. Woke up client again and gave her 1/2 of vicodin passed 1 pm (before 1:15 'sh pm) and told her my call to pharmacist. And told her no Ibufrofin for her for the night, but told her to take 1/2 of the vicodin on the bowl at passed 7 pm and passed 1 am, told her there's no more Ibufrofin on the bedside table for her. Changed tv channel from QVC to 8. Massaged client's back with muscle cream. Was about to invite me to stay overnight, but told her I wasn't even feeling 100% well yet. But asked her if she needs someone for the night, I would let office know, says no, she'd be fine she thinks.Left 1:30 'sh pm. Texted her son about pharmacist talk and texted it to Merrille as well.
Jan. 15 (Wednesday) = Dog outside. Client soundly asleep, s didn't woke her up. Tidied up kitchen. Laundry. Client awake almost 10 am. Hadn't heard complain f back pain. Says she took 2 halves (makes it 1 while tablet) of vicodin maybe 2 hours before I came in. Son called me and asking for client's condition. Told him she's still coughing and congested. Says he will call Scripps home care and see if they could come and see her. Tv on. Massaged client's back, rubbed. Phone call, on land line, from Scripps home care nurse, asked about client's condition. Client wrote 3 checks: Smart & Final, Vons, Rite-Aid. Called her bank, and spoke to a male employee, but not Carlos. Says she was asked if she would like to transfer $3000, but she said $500. Nail trimmed. Left before 11 am. Went to Vons first, got Rubitussin, called client about chicken price, says better get it from Smart & Final. Paid by check. At Smart and Final next, got more dog foods and taffy candy and called client about chicken, decided on chicken leg and thigh for a little over $5. Paid by check. At Rite-Aid and got more muscle rub and cheese chips. Paid by check. Got back a little passed 12 pm, client lying in bed, asked for soup and muffin only she says, to save some time. Got call from client's granddaughter, says she heard client took 2 Ibufrofin tablets, and I said yes, yesterday. Then passed phone to her dad, asked me about client's condition, and says he had spoke to Maury too, the nurse I spoke on the phone and asked if I'd be here for the afternoon if they could come. Client had it for her lunch. Prepared meals: corned beef sandwich, sauteed beef strips with some spinach leaves and half baked potato, also two cheese blintzes with other half of the potato. Called Merrillee and told her. Client had 10 ml. Rubitussin 12:22 'sh pm. Tidied up kitchen. Folded clothes. Emptied commode. Client muscle rub, hot water, rub. Texted her son 1:30 and asked him about update on his callas I hadn't heard from him yet, told him I could only stay until 1:30 to 2 pm. Got a call back from him 1:45 'sh pm, says again, he will call Scripps homecare, an told him I need to b home 2:30, mom works at 3 pm. Told him office could just send another girl, asked me to not call office yet, just hold it off and he will call them first and see from there. @ pm, no call yet. Called client's son, ringing, and then the static. Called Merrilee, voicemail. Called son again, says voicemail is full so I can't leave voicemail. Called Merrilee again, and spoke to her about situation, asked about client. Told her client still coughing and congested, son trying to have physician come over as he was afraid client will get pneumonia. Merrilee said I could go, tell client to call office if they need someone there. Told it to client, and explained the vicodin on the bowl, time of taking next one, 7:30 pm, and 1:30 am, every 6 hours. Client watching tv and having taffy. Changed her her gown. before grocery. Ad told her about her son trying to have Scripps homecare physician and nurse there to check on her. Left already 2:20 'sh pm. NOTE: 8 halves of vicodin (4 tablets.)
Got a call from client's son when I'm home and asking if I got a call from Mary, I said no. But saw a missed call when I got home, but no hadn't left a voicemail. Says it's too late now for today, told him physician could just come and office will send another girl, and I could talk on the phone to them. Then again says he's busy, taking care of this, and that, has to do this ,etc etc. Says will call Mary again and see, and wants me to stand by. Called Merrilee and told her.
Jan. 16 (Thursday) = OFF!
Jan. 17 (Friday) = Back door locked. Client awake. Client's bedroom window above bed was open. Client says girl last night left it open as it was hot. Client still congested and still coughing. Put cream on back, then hot water. Client had tea and a waffle. Took her Hydrochlorothiazide and Vicodine (1 tablet) before 10 am. Changed client's dress. Talked to me about the girl that was there yesterday and last night (Karla) and Merrilee was there too, and that she was surprised the mobile physician (Jimmie Thompson) was there, and she was in her room. Thought girl had her Smart & Final check and afraid that girl could cashed in money, or used her check. I found the Smart & Final check, (dated yesterday (Jan. 16) but client say it was written poorly, she could have written another check. Worried. So I told her I'd call Merrilee and asked about it. Called Merrillee, and asked, said Karla, (the other girl) didn't went to store yesterday at all, but the other check client wrote for her was to pay Karla $8 she spent to pick up client's prescription. Told it to client, that the check that I found on the floor in her bedroom was indeed the check she wrote for Smart & Final, and the another check she said she wrote was for payment, for Karla when she paid for picking up client's prescription. Client wrote 2 checks: for Rite-Aid and Smart & Final . Got call from Mary, Scripps, RN homecare services. Asked me until what time I'm at client, told her until 1 pm, so she said she'd be there 12 noon. Client had Azithromycin (antibiotic on the fridge) 2ndday today, before 11 am. Left for stores 10:50 'sh am. At Smart & Final first, got items, called client for Hebrew National footlong hotdog's price, $9.99, client said it's ok so I got it (10 pcs. in a package.) Paid with client's check. At Rite-Aid next and got over the counter Guafenesin 5R 600 mg, and hydrogen peroxide. Paid with client's check. Got back passed 11:30 am, client watching tv. Prepared client for Scripps RN's visit at 12 noon. Client at commode and tried to get grab onto her walker after as I keep on telling her it is better in the living room to go. Tired and says all she wanted was to sleep. So told client if she feels that way that she can't go to the living room, she could just stay in her bedroom. Client fall asleep. Preparing soup and boiling 2 ft long hotdogs, accidentally sent an unfinished text to client's son about what meds. client had taken. Son called, talked to me about yesterday and office, Merrille, the meds. etc. the antibiotic that client took 2 tablets on the first day. Client had a cup of mushroom soup, consumed almost half. Called Merrilee and told her, she called Karla and confirmed. Then called me back and says she talked to the pharmacist and it was 2 tablets on the first day, together. Told her also about the Scripps RN coming that client said. Scripps homecare service, RN, Mary R. arrived passed 12:40 'sh pm. Got client's medical history, and asked client some questions. Got client's vital signs, temperature, 97, BP, 123 (or is it 120?) / 57. Client had hotdog sandwich, only consumed half of it, says hotdog was hard, buns not good. Client had Guafenesin tablet and Atenolol 2 pm. (Left 1 Atenolol) at kitchen for later with a note, "tonight. Got a text from Merrilee, "how did it went?" and I replied "Not done yet, RN arrived before about 12:45 pm, but still here." Was about to text son about me having to leave, already 2:33 pm as got a call from my mom asking where I am, I'm using her car. I got my car fixed BUT brother used my ca, and mom knows I'd be home before she lives. So mom had to call in she'd be late 30 minutes. Son called me just when I'm about to text him, asking about updates how it went, told him RN, Mary's still here, so he spoke to her. RN left passed 2:35 'sh pm.Put half of sandwich back on fridge and put the halves of the vicodin (3 tablets) on bowl, on bedside and explained it to client, time of taking, 4 pm and 10 pm for the night.) Left 2:45 'sh pm.
Jan. 18 (Saturday) = Backdoor locked. Client awake. Hadn't heard complaints with back pain. Says she feels better, better today than yesterday. Still congested and coughing, but mucus loosening up, a bit hoarse. Muscle rub on back, says back itches. Tidied up kitchen. Made client breakfast. Client had tea and 1 sunny side up. Client had her Hydrochlorothiazide and Guafenesin at 9:47 am. Says she had 1/2 tablet vicodin about 30 minutes before I came in at 9 am. Massaged back again with muscle cream, rubbed, hot water. Changed client's gown and changed bedsheet. Soaked soiled linens. Turned on tv. Client fall asleep. Had prepared meals. Prepared 2 cheese blintzes, and rice-a-roni beef rice with sweet corn. Client awakened, still wants the Ralphs Greek salad and cole slaw to be returned. Client had Azithromycin @ 11:22 'sh am, before leaving for Ralphs. Got graham fudge, Greek salad and can of sweet corn, and UT newspaper. Back 11:45 'sh am. Continued preparing the meals. Checked on client as hear her kinda growling, going back to lie down in bedfrom commode, scratched back. Made steak for lunch. Client had steak and Greek salad and water for drink for lunch passed 12 noon. Prepared meds. 3 tablets of Guafenesin good until tomorrow, Sunday until I come in Monday. 7 halves of vicodin. BP mes on kitchen table as usual. ALL with notes on how often meds are taken. Awakened client 12:50 'sh pm and gave her Atenolol and explained meds and put vicodin and Guafenesin 3 tablets on separate bowls with the notes. Client wrote check to pay Noelia for the Chinese food last week's Saturday, $5.50. I got the check, told client I will ask office for Noelia to have it. Left 1:10 'sh pm.
NOTE: two halves of vicodin (1 tablet) left by client on bowl along with taffy candy wrappers. Texted Noelia about the check from client, say hold onto it 'till we met again.
Jan. 19 (Sunday) = OFF!
