Jan. 27 - Feb. 02

Jan. 27 (Monday) = Turned on dryer and laundry as soon as I got in. Medicine for yesterday (hydrochlorothiazide and Guafenesin and Atenolol) was untouched.Client awake. Back pain. Says she took pain medicine last night when asked what time she took it. Then says what she took this early morning was the green med. (which is for the itch.) Asked her again and says she took white meds. early morning (explained it again to her the white ones are or pain and the green ones are for the itch.) Says he took the white ones last night. Wants muscle rub on her back so put muscle rub. Then says her back is burning. Used towel on back. Sponge bath. Changed dress. Had tea and a waffle, took her hydrochlorothiazide and had 1 Ibufrofin and half of vicodin for pain at 10:30 am. Swept bedroom area. Emptied commode. Told client RN, Mary is coming today to visit. Client don't want it and asked me to call RN not to come. Had a voicemail from RN about her visit. Called RN back and told her client doesn't want the visit today. Asked about client condition, told her client was in pain when I came in and just wanted to rest as she was awake most time last night. Says she will just come on Wednesday instead. Told it to client, but still client don't want the visit. Tidied up kitchen area.Defrosted 1 chicken leg for lunch. Changed bedsheet. Changed towel rug. Tv turned on. Client fall asleep. Prepared lunch. Client had lunch passed 12 noon. Had grilled chicken leg with ketchup, half of baked potato and sweet corn and water for drink. Liked the chicken. Had Atenolol. 4 halves of vicodin (2 tablets) prepared for later and put on client's bedside table with 2 hydroxyzine (remained from yesterday) on another bowl. Told client she could take the pain med. 4:30 pm. Call the time to check first and meds. are every 6 hours. Checked phone: working. SAys she have to wait for her check before paying my office. Changed bedroom's light bulb before leaving. Left 1:15 'sh pm.

Jan. 28 (Tuesday) = Continued laundry and dryer as soon as I got in. Client awake, sitting in bed. Says hadn't taken pain meds this morning, but took them last night. Client had tea and fried egg. Had 1 Ibufrofin and a half of vicodin 9:30 am. Tidied up bedroom. Turned on tv. Client fall asleep. Cleaned kitchen floor, tidied up kitchen. folded clothes. Changed clients dress. Client had lunch passed 11:30 am. Had chicken sandwich on Hallah bread, with tomatoes and pickles and water for drink. Says dog had some of her lunch. Still saying about the pain and asking if she could take more meds. Told her not time to take more meds. Swept bedroom carpet. Watered plants on the front door. Got Scripps mail that says put on the Scripps folder until clinician ready to fill it up (put it in Scripps folder.) Client asleep after lunch. Had her Atenolol med., about 12:35 'sh pm. Cut toe nails. Brought date nuts to her room and explained her pain meds (4 halves of vicodin and 1 Ibufrofin tablet.) Asked for the green pill so put 3 of those too on the bedside and explained again it's for the itch and white pills for the pain. Says she is itching when asked so she took 1 hydroxyzine just before 1 pm. Told her she could take pain meds at 3:30 pm. Left 1 pm.
Got call from Scripps home care case manager Sandy, asking about RN, Mary's visit ad client's condition on my way to the office.

Jan 29 (Wednesday) = Dog outside on top stair, coughing. Client awake. Back pain still. Says took meds last night by 6 pm. Had tea and waffle. Had her hydrochlorothiazide and Ibufrofin and half vicodin about 9:30 am. Hot towel on back ad had sponge bath. Turned on tv. Did more laundry. Changed client's dress. Changed bed sheet. Client thinking of Marie Callender's for lunch. Client had fallen asleep 10:30 'sh am. Scripps RN, Mary says she will be visiting between 11 am  to 12 pm. Prepared grilled chicken. Client had lunch passed 12 pm, had grilled chicken leg and thigh, (cut it in bite sizes for her), steamed zucchini, some Nacho chips and water for drink. RN arrived passe 12:30 pm. Vital sings ok. Talked with me about client's supposed to be "s." But to nurse, it's just wasn't "s," just cant be asclient hadn't been outside, but more likely flea bites. Tried to call moile physician, called their office, told me P.A.Jimmie Thompson will call her. Says she told them if maybe he thinks it's flea bites as the skin scrapping wasnt done. RN left passed 1 pm. Client had Atenolol and Guafenesin. Told her the refill ready from Rite-Aid pharmacy (Ibufrofin.) Left 1:10'sh pm to pick up refill. Paid with check. Prepared 12 Ibufrofin tablets for client, also about 4 hyroxyzine as well, with note for both on what it is for and how often to take, with tomorrow's other meds on kitchen table. Refilled hydroxyzine. Left about 1:30 pm.

Jan 30 (Thursday) = OFF!

Jan 31 ( Friday) = Dog barking, lease stocked on door and hadn't been out yet. Client awake, lying in bed. Says about her son'scall last night, about mobile physician's office called about her vital signs ok, but a bit high on cholesterol. Back pain. Says took meds. and feeling nauseated. Client had tea, took her hydrochlorothiazide and 1 tablet of vicodin, then had a waffle while I put muscle cream, and used warm towel on her back after as she asked. Turned on tv. Washed soaked towel rugs. Continued comforting and using warm towel on client's back for about an hour. Made list for Vons. Called Marie Callender's for lunch menu as client thinking of it. Client spoke to Carloson her bank to check her balance, says she have over 1K in her checking. Wrote check for Vons. Decided to skip Marie Callender's for today.  Left 11 am for Vons. Gor items and called client for lunch $5 Friday they have. Decided on Burger King instead. Paid groceries by check. At BK next and got double cheese burger ad value fries. Spent $3.22 with my card. Arrivd back at client 12 'sh pm, warmed up fries and mae steamedzucchini and broccoli. Client ha lunch passed 12 noon. Had double cheese burger, steamed zucchini and broccoli, fries and Diet Coke for drin. Only had half of the sandwich but ate the slice of Boston cream pie. Says she will finish sandwich and the est later. Client sleep. Had her Atenolol before 1 pm. Says sandwich wasn't that good. Prepared 2 tablets (cut in halves) of vicodin set on her bedside table. Told client she could take vicodin by 3 pm as she took 1 9 am. Left passed 1 pm.

Feb. 01 (Saturday) = Client lying in bed, awake. Back pain. Client had cocoa and fried egg while im using warm towel on her back. Then asked for muscle cream and says it burns. Used cold towel. Took her hydrochlorothiazide and took 1 ibufrofin and half of vicodin at 10:11 am. Turned on tv. Changed bedsheet, and changed clients dress. Swept bedroom and emptied commode. Laundry continued. Changed towel rugs. Tidied up kitchen. Prepared lunch and tomorrow's meal. Made sliced hotdog, fried with sweet corn and steamed broccoli. Grilled salmon fillet. Client had lunch a little passed 12 noon: had 2 salmon fillets, baked beans and steamed zucchini and diet coke for drink. And served her small slice of boston cream cake. Client ate all, and likes the fish. Prepared meds. Client had atenolol, 1 pm and explained her how often the ibufrofin could be taken, every 8 hours. Put it on bedside table. Left passed 1 pm.

Feb. 2 (Sunday) = OFF!
