Sept. 24 (Monday) = Arrived and client told me about trying to call me last night but can't contact me so she called Merilee instead. So that's why I have a missed call from her I thought to myself, but didn't have a voice message. Client told me about the notice of complaint she had read from the mail I had set on the kitchen table. The date on the noticed was Sept. 19 and had a remarked of client has to call the selected agency number in there until Sept. 20, we hadn't found about the noticed until Sunday. I tried to call the number but was closed. Went to Subway and had tuna sandwich for lunch. Also got the dry dog food from Pet Market for $14, already with tax. Gave client her morning medicine too, made tea. And later gave the other medicine before I left. Both are for high blood pressure. Made baked potato for her and spinach. Got to Rite-Aide and got telephone cord, M & M's, Mrs. Field's cookies.
Sept. 25 (Tuesday) = Asked me to call the number again from the notice when I got in. Tried it but still closed but was open today starting 9:30 so I said I'll just call again at 9:30. Made coffee, put dog out, laundry, washed dishes and wipe some parts of the floor and cleaned the mess. Gave her morning medicine and gave coffee and then I tried and call the number again. Got hold of someone but said she can barely hear me so I said I'll try and call again. unplugged and put the new cord back, and called again. Got a hold of someone (another lady) and this time it was better as I didn't got hearing that I was barely heard. She asked me the name and address, spelled it. And when asking what best day someone can come I gave the phone to client and she talked to hear. Client handled me the phone back when was asked about the day of someone coming there. I said I'll be there tomorrow and Friday 9 - 1. And said someone is coming there either tomorrow or Friday between 11 to 1 pm. Went to Subway passed 10:30 and got oven roasted chicken and I paid for it with my card for $5.39. Client had it for lunch and made french fries, had water for drink.Gave her the other high blood pressure medicine before I left. Emptied the commode as well, left dog food on his plate, and made tomato soup ready on a cup on the fridge and squash, and left some french fries on the table.
Sept. 26 (Wednesday) = Dropped by to Rite-Aide and got Diet Coke and Alka Seltzer and spent $4.93 with my debit card. Then went to Popeyes and got Dip'n Chicken for lunch, read it from their ads for $3.99 with biscuit and fries. Got back before 11 am and made lunch ready. I re-heated the fries on the pan as it was cold and the biscuit on the toaster. The chicken pieces are cold too so I also re-heated it on the pan. Made spinach and brought it to client. Client noticed it was still cold so I asked her if she wanted me to reheat them, and agreed. Then later heared her from the kitchen that she wanted to return the Di'n Chicken instead. I went back to the room and confirmed it and returned the food on Popeyes and got a coupon instead of 2 pieces of chicken for $1.49. I then got it and got 1 apple pie for .99 cents. I still had some changes of $3 and something and kept it as I used my debit card to pay with the Dip N' Chicken.Got back and gave it to client for lunch, and half of the apple pie and water for drink. Didn't gave meds. as she said she wanted to try not to take it as she thought its making her feel bad. Client owes me only $2.67.
Sept. 27 (Thursday) = OFF!
Sept. 28 (Friday) = Went to Rite-Aid as client asked me to get milk, and small size chocolate (Snickers, .97 cents.) I paid for it and had spent $3.36 with my card. Picked up an order from DZ Akins at at 10:30 am that client had ordered: Hallah bread, square, small dill pickle, 1 cheese pocket, roast beef, 3 plain bagels, 1 small container of cheese cream. Client asked me to pay for it first and so I did using my card and spent $14.81. Made roast beef sandwich for lunch, 1 snicker (which she didn't touch,) some fruits (cantaloupe and watermelon, she didn't touched as well,) and water for drink. Client had shower at 12:30 and asked me to rub her pain cream (Icy Hot cream). I left already 1:15 pm.
Note: Animal shelter officer, Edgar arrived passed 12 noon and checked the dog and saw doggie wasn't limping as complained said. Asked for the dog to be groomed and told either her or another officer will be back either Oct. 5 or Oct. 6 to check the dog if groomed, same time 11 am - 1 pm.
Sept. 29 (Saturday) = Been trying to call client if she wants me to drop by to Food 4 Less for groceries tomorrow is Sunday but can't contact her, phones goes "beep...bbeep..." Got a hold of her while I'm driving and told me not to so I didn't. Changed bed linens, pillow cases and blankets when I arrived. Went to Rite-Aid and got Diet Coke, Tylenol and Snicker for .97 cents and spent a total of $11.72 with my card. Then went to Subway and got 2 footlongs, 1 tuna for $5.00 and 1 roast beef for $ 6.95. Used my card and paid for $ 12.88. Then to Vons to get small dessert and Foster Farms BBQ chicken wings. Had spent $10.98 for two items. Client had tuna sandwich for lunch, strawberry shortcake for dessert, 1 snicker, and a cup of soda for drink. Gave her 1 blood pressure med. after lunch. Also I called the Pink Poodle salon and set appointment for JR (the dog) for Thursday next week.

Sept. 17 - Sept. 23
Sept. 17 (Monday) = Client awake, sitting in bed when I arrived. Room messy and full of papers all over the place and the kitchen. Said she hadn't had a good day yesterday, as the dog wasn't that good and the arthritis pain. Cleaned the messy papers and separated papers in the trash. Emptied trash and rinsed. Made client coffee and give morning med. and she noticed front lawn as she sits there and wants it to be watered. So I said I'm going to be outside o water front lawn while the dog was outside as well. Was supposed to write check and asked me if Rite-Aid accepts card, and I said yes and so she wanted and asked me instead to used my card and she would just pay me. Went to Rite-Aid for some items client wants on her list: ear oil, hydrogen peroxide, Mrs. Fields cookies, soda, and candy bars. Got there and got Sprite worth 99 cents, doesn't have ear oil and out of hydrogen peroxide. Paid the items I got using my debit card and spent $5.32. Then next to Burger King with a Whopper coupon and ordered 1 Whopper and onion ring. (Got 1 Whopper free.) Used my card and spent $ 7.52 but wrote down $6.92 for client. Client had Whopper for lunch with soda pop, onion rings and gave 2 candy bars. Gave the rest of the meds. afterwards. She called her bank to verify is she has enough money as she has to write a check. Left at already 1:20 pm.
Sept. 18 (Tuesday) = Client sleeping, woke her up. Put doggie outside on leash. Cleaned mess/crumpled paper on the room. Wiped dust on room window and above the header of the bed. Made tea and gave morning med. at past 10 am. Left for Popeyes at past 10:35 am with a coupon of $2.99 for 2 pieces of chicken, 1 regular side and biscuit and two apple pies for 99 cents which she had for lunch with Sprite. Used my card again and spent $5.36. Gave rest of the meds. afterwards. Changed bed sheets and put towel under as doggie throw out in there, mostly water. Did laundry and and drying. Emptied commode. Left at past 1:10 pm. client sleeping in bed, just woke her up to tell her I'm leaving.

Client didn't asnwered the phone the first 2 attempts I called, picked the phone the third time but I was having a hard time hearing her voice as I can't hear anything just some static, same when I called the 4th time. Then she called me instead twice but can only hear her a little bit, still I can. Gave me a list of what she wants such as milk, dog food, fruit, veges. just those. And asked me to get some things else that we need when I saw it on the store. Got there about 9 am and got all the items and also got her soda pop, 3 tv dinners, 2 canned of soup (1 chicken noodle and 1 tomato soup.) Used my card and spent $13.83. Then dropped by to Subway on my way and got tuna sandwich. I paid cash since I don't want to used my card as there's not much in there. Spent cash on Subway for tuna : $5.39. Cleaned the mess, papers on the kitchen floor and on client's room when I got there. Also dog pooped on the hallway going to client room so I shampooed the carpet too: the hallway and the vacant room close to the kitchen as the dog peed in there as well since the past days.Tried the client restroom but still clogged so I emptied her commode on the other toilet bowl near the washing machine and dryer. Had tuna sandwich for lunch, Coca Cola for drink and steamed broccoli and pieces of cantaloupe. Didn't give her the bladder medicine as she asked to try not taking it as she said its making her go more. Only gave her the blood pressure medicine which the other one was on the morning, and gave one before I left. Left at about 1:15 pm. Also had the phones ready for return, the one I got from Big Lots almost 4 months ago, and brought it home with me to drop by to Big Lots and try and return it.
Sept. 20 (Thursday) = OFF!
Sept. 21 (Friday) = Dropped by to Big Lots with the phone client wants to be returned. Arrived there minutes before 9 am, before the store opens. Got in and tried to return phone but associate won't accept it as it's been too long ago. Got to client's place and told her and was mad to the store and called them. She ahd spoke to John which is the manager and was told to bring back the phones tomorrow and she would be given a gift card. Got to Subway at 11 am and got oven roasted chicken for lunch and I had spent the money using my card, sandwich worth $5.39. Gave client her one blood pressure medicine (white one.) She doesn't want to take the one for bladder for the meantime as says it keeps her going. went to her room 4 minutes before 1 pm and she was looking for pain cream and so I suggested I could go quick to Rite-Aid to get her one before I go as she needs it. She then asked me if I could charge it into my card so I agreed. Went to Rite- Aid and got the cream and wanted me to get nail polish remover too. I used my card and spent $9.49. When I got back she asked me if I got the M & M's and I said not. Said she called Rite-Aid and associate told her it would be ready on the counter. Got the phone with me again so I can drop by to Big Lots again tomorrow.
Sept. 22 (Saturday) = Dropped by to Big Lots to return the phone, store had just opened and manager (John) was there. Told her maybe I could exchange the phone and so I went and look but there's no kind of phone in there like the client's phone in her room. Was about to give me gift card but managers told the associate to just give me cash ("never mind... just give her cash.") And so I got cash back which was $43.09. Got to client's place and gave her the money and said she was thinking about Food 4 Less and I agreed. Told her I could go in there as she doesn't have some food for tomorrow, it's Sunday. Told me to make eggs first and eat before I go. I went and made an over easy egg but only for her. Told her I'm going to Food 4 Less after few minutes and so I went. Gave me $21 and a list on what to get. Got small watermelon and BBQ chicken wings (Foster Farms.) Then went to 99 Cents and got plastic wrapper, 3 Cracker Jack, cooking oil and egg. Also grab egg for me and 3 coconut drink and paid separately using my card. Also had returned the dog food I got last time I went there as doggie won't eat it. Got back to client past 11 am and made 2 biscuits for lunch, and had chicken wings and made her squash and gave water for drink. Gave 1 blood pressure medicine few minutes before 1 pm. Left at about 1:15 pm.

September 10 - September 16
Sept. 10 (Monday) = Client sleeping. Put doggie out and while waiting, watered lawn. Separated papers on the trash and emptied it and washed trash bin. Refilled doggies water and washed the white bowl. Wiped table and washed dishes and wiped half of the kitchen floor. Gave tea to client before or about 10 am with the morning med., talking about the dog being bad yesterday and last night. Noticed the spot on the vacant room, doggie just went in there last night probably, or early morning as it's still looks fresh. Client talking about her arthritis pain, like pinching and she's out of pain cream so wants me to go to Rite-Aid and get some. Also said the $1 worth lollipop was no good. Asked her if she wants me to get 1 bag of the lollipops I was talking about that's worth $3.49 (or something) and comes in different fruit flavors and she thought she'd better try it instead. Wrote me a check for Rite-Aid and I went before 11 am, about 10:45 or so. Got the pain cream worth about $5 and some cents Rite-Aid brand and got 1 bag of the lollipops. Then went to Subway and got the tuna sandwich. I paid for it and used my card, spent $5.39. Client had the other half of the tuna sandwich, water for drink, and lima beans. Gave her the other 2 meds. about 15 minutes before 1 pm. Also told her I left 3 tablespoons of Milk Of Magnesia on the table along with 1 cup (8 oz.) of water for her. She could take it about later in the afternoon or fort he night. Asked her about food for dinner, suggested I would make egg. But she said no, she would have soup instead, so told her there's still tomato soup on the ref and I'll put it on the green cup so would be easier for her to take it out. :) Wrote a check for the agency and called her bank to make sure she has money on her checking account. Left 1:06 pm :)
Sept. 11 (Tuesday) = Client sleeping. Went back to sleep after I woke her up and woke up before 11 am. Decided to go for Burger King for lunch and used my card. Also was discussing me about the money she owes and maybe send me to the bank tomorrow or another day to pay me. Went to Burger King passed 11 am and got 2 cheese burgers, small onion rings, and small strawberry shake. Had spent $7.07 with my card. Had it for lunch and water for drink. Gave client 2 of the meds. except the one in the morning as I forgot it. Made barley and mushroom soup for dinner and put it in a green mug on the fridge. Went looking for blankets that she was looking for at 12 noon but didn't found those. Left a little past alauna.

And again, as always, wants the bread back in the store. And I told her it was already expired. Still want the bread back in the store, so I said I would try if they would accept it back. Morning med. was taken. Made an order from DZ Akins before 10:14 am and I went to pick it up before 11 am. Used my card and spent $18.67. Got bread, 2 cheese pockets, dill pickle, cream cheese, 2 bagels and roast beef. Made sandwich for lunch and spinach for client and had water for drink.Gave the rest of the meds. Shampooed carpet as doggie peed in there again. Looked for blankets that said to be missing on the bottom of the cabinet on the vacant room near the kitchen. Client sleeping in bed when I told her I was leaving. Made her another sandwich, and steamed broccoli for food and stock it in the fridge. Left a little past 1 pm.

Sept. 14 (Friday) = Called client in the morning and asked her if she wanted me to get something from Food 4 Less as I was dropping there to return the expired Wonder bread. Answered the phone on the third time and gave me a list. Arrived on Food 4 Less before 9 am. Left about 9:30 and tried and return the bread. Accepted it back and register said "I'm gonna accept it this time." Bought veggies such as Italian squash, potatoes, carrots. Also got 3 pieces bananas, 4 soups, 1 tuna, 2 dog foods, salad bag, 2 tv dinners, Diet Pepsi fro drink and carrot cake for dessert. Used my card and spent $18.18. Then called client to tell her I'm going to dropped by to 99 Cents to get her Cracker Jack. Got there but no Cracker Jack. Got cleaner instead, 1 paper towel, potato chips and milk bones as client requested. Used my card again and spent $4.23. Arrived at her house at past 10 am client sleeping, woke her up and put doggie outside. Had beef bacon sandwich for lunch, veggie salad, slice of carrot cake and soda for drink. Gave her one of the blood pressure med. and bladder med. Left at passed 1.

Emptied commode but had lots of paper towels and tissue papers that clogged up the bowl. Tried flushing it few times but didn't work although the papers had been out, pulled it with the grabber and put on plastic bag. Got the commode back to empty some water on the toilet and disposed it on the other bowl. Tried flushing again and didn't work. Redid the water and emptying it on the other toilet bowl. Still clogged thou. Client wrote a check for how much she owes me ($68) and called the bank and spoke to someone named something like Alicia (?.) Went there and the lady was there and was the teller who helped me, Fhines was there as well and instructed the lady to give me cookies. Went to Burger King passed 11 am with a coupon for Whopper but Burger King wasn't open yet until an hour more, went to Subway instead and got tuna sandwich. Paid using my card and spent $5.39 Client had half of the tuna sandwich as we split it in half, water for drink, 1 cookie from the bank and the coleslaw I had brought with me, which she hadn't touch I believed as it looks untouched when I got the plate after lunch. Gave her two of her meds. after lunch: for the bladder and 1 for the high blood pressure. Prepare shower @ about 12:15 but hadn't been on the shower until about 12:20 something. Also gave bath to the dog with the client and helped me with the dog. Showed her the 2 tv dinners that was on the freezer, the steamed broccoli on the fridge and the half of the cookies on the table as she wants me to take it with me, told her I'm just going to get half instead so she could still have some left. Left at about 1:20 pm. (NOTE: Was paid $68, amount she owes me for groceries and lunch as client would asked me to pay for it first. Then owes me a new one which was $5.39 for tuna sandwich for lunch)

Received a call from client before 9 pm ans was talking again about the phones has to go back.... And some caller reaching for his husband. And a lady later on asking like ,"what kind of phone you have anyway?" And that she hung up the phone and receiving a call again with a lady looking for someone.And said she thought it was my mom and my mom was looking for me. And so she called me as she got worried.

Paid using my card and spent $5.39 Client had half of the tuna sandwich as we split it in half, water for drink, 1 cookie from the bank and the coleslaw I had brought with me, which she hadn't touch I believed as it looks untouched when I got the plate after lunch. Gave her two of her meds. after lunch: for the bladder and 1 for the high blood pressure. Prepare shower @ about 12:15 but hadn't been on the shower until about 12:20 something. Also gave bath to the dog with the client and helped me with the dog. Showed her the 2 tv dinners that was on the freezer, the steamed broccoli on the fridge and the half of the cookies on the table as she wants me to take it with me, told her I'm just going to get half instead so she could still have some left. Left at about 1:20 pm. (NOTE: Was paid $68, amount she owes me for groceries and lunch as client would asked me to pay for it first. Then owes me a new one which was $5.39 for tuna sandwich for lunch)
Sept. 16 (Sunday) = OFF!
Received a call from client before 9 pm ans was talking again about the phones has to go back.... And some caller reaching for his husband. And a lady later on asking like ,"what kind of phone you have anyway?" And that she hung up the phone and receiving a call again with a lady looking for someone.And said she thought it was my mom and my mom was looking for me. And so she called me as she got worried.

September 03 - September 09
Sept. 03 (Monday) = LABOR DAY.
Arrived with client awake in bed. Told me that she is having over indigestion and that she had called Rite-Aid earlier but thought they were still close at the time. Made client a cup of tea. Then later on went to Rite-Aid to get something for indigestion, 3 Bridge Mix chocolates as it was on sale, Mrs. Fields cookies, 99 cents soda pop. Asked the pharmacist on what to get for indigestion and suggested Emetrol. Got Emetrol and worth $9.99 cherry flavored. Gave 2 teaspoons to client when I got back. Told her no drinks before or after but she still took a sip of the tea. Went back lying down and I continued my work. About before 11:30 am, I started making fries, spinach for veges. Client woke up just before 12 noon and said she's not planning on having lunch, but she would have soup instead. Reheat Chicken Noodle Soup on the stove and gave a cup to client. She also asked for a piece of bacon and gave her a few pieces. Also asked for soda pop later on, which had help her feel better with her indigestion. Gave her blood pressure meds. later on. About before one reminded client about calling her bank and making sure she has enought money on her checking account to pay the agency. Tried calling the bank to make sure it was really close, and it was closed. Called the office phone number and asked if it was ok to bring the check tomorrow and says it was ok. Left at past 1:15 pm.
Sept. 04 (Tuesday) = Got in there and client awake, said her indigestion was back again. And the Emetrol syrup was no good, she wants it back to Rite-Aide. I went to the kitchen and noticed the dried spills in the kitchen table and on the floor too. Client had a cup of tea. Went back to sleep. Had tuna sandwich from Subway for lunch, then went to Vons and bought 7 Up Diet, and got coleslaw from Popeye. Client's $10 has a change of .38 cents. Client had the half of the sandwich, a cup of 7 Up soda, and coleslaw for lunch. Later said she's better, the soda had probably helped. Informed me Rite-Aid called about the refill ready for Vesicare medicine. Took blood pressure medicine after lunch and watching her show at 1 pm. Changed bedsheets and blankets today.
Sept. 05 (Wednesday) = Arrived client was on the kitchen and the dog was out. Cleaned the cabinets and ref. and disposed of items that needs to be disposed off. Threw away some beans that looks no good on the bottom cabinet and all the jellies. Had tuna sandwich again from Subway for lunch. Client asked me to pay for it first and just list it so she knows how much she owes me. Went to Subway and used my card and cost me $5.39. Then I went to Vons and get the cole slaw from the deli worth about $2.99 just like the employee has told the client as she called the store and asked. I also got 2 tv dinners as I was off tomorrow. Spent a total of $4.78 with my debit card. Dropped by to Rite-Aid and got Alkaziltzer for client, used the check she wrote for the store. Client had half of the sandwich for lunch, and cole slaw. Had water for a drink and had taken blood pressure meds. afterwards. Also had reminded her about the Vesicare that was ready for pick-up from Rite-Aid Pharmacy. Asked me how much it's worth and when I told her $42 she said it was costly and wants to call the doctor (mobile physician) and that maybe he could give her something else, something less costly. Tried to call the number but no one's answering the phone. GOT A COLLECTION NOTICE FROM THE MAILBOX.

Sept. 07 (Friday) = Arrived before 9 am and dog was outside, client was on the kitchen. Told me about the alkziltzer and that she thinks it looks different from the original one and she thought medicine changes so maybe it was a new one or a new kind. Tried it but didn't helped. So she called the pharmacy and asked and learned it was the wrong one as it was for cold and flu. Told me she has a stomach ache and instead of coffee or tea she wants to try and have soup, maybe it would help. I heated tomato soup on the stove and gave a cup to client on her room. Reminded her about the pick-up of the Vesicare medicine and that she only have until tomorrow, Sept. 08 for it to be pick-up. She said she forgot to try and call again the doctor and asked me if I wanna call. I did called the number and had spoke to Tanya. The call got cut off and I called back again. Spoke to the same person and later to another lady named about Sharon (?) (there's "sha" on the start.) Asked me the client's full name and later the Rite-Aid's phone number and told me they will call the pharmacy and call me back.
Gave the client the collection NOTICE while we were waiting for them to call back. Client wasn't happy about the notice and decided to call her husband and told him. Done on her phone call at about 11 am. I tried and called the mobile's physician number again as client is anxious as to what had happened but nobody's answering the phone. Then told client I have to go to Rite-Aid and return the alkaziltzer and she wants me to get something or whatever the pharmacist will suggest to help with elimination as she said she hadn't had B.M. for 4 days. Went to Rite-Aid and asked the pharmacist and suggested the Mucilax. I got the small bottle only worth $8.99 and returned the alkaziltzer. With the deduction of the returned item, I paid only $1 and some cents with clients check. Then went to Popoyes and got 2 pieces of chicken with a biscuit, and 1 apple pie. Used my card and cost $4.29. Client had the 2 pieces of chicken for lunch, the biscuit, lo mein that I brought with me, and water for drink. Gave her the blood pressure meds. afterwards. Also tried to give bath to the dog as client requested but doggie won't cooperate, doesn't want to get into the shower. Prepared the Mucilax powder on 8 ounces of water and just left it on the kitchen table for client to take later in the afternoon or early tonight. Left client's house at about 1:15 pm.
Sept. 08 (Saturday) = Client said she was planning of calling the attorney's office about the collection notice she received. Told me that she called her husband and her husband told her not to give even a penny tot them. Told me also about the call of her granddaughter who said they we're probably coming tomorrow (Sunday) to visit her and client doesn't like it. Wrote me a check to use for Vons as I insisted she doesn't have food for tomorrow and it's Sunday. Then she gave me $4 and some cents for Burger King burgers. Went to Rite-Aid first to pick-up client's Vesicare for bladder which cost $42. She wrote me a check but before that she called the pharmacy and spoke about her concerns about the price of the medicine and that if it would help her situation. Went to Vons and I bought 3 tv dinners, ginger ale, and 2 dog foods. Then went to Burger King and got 2 double cheese burgers and small onion rings. Client had the cheese burgers for lunch, and onion rings, ginger ale. Also gave her a 1 Mrs. Fields cookie but she didn't touched it. Gave clients blood pressure meds. except the Vesicare before 1 pm. Cleaned everything that needs to be cleaned for the day: kitchen, trash can, dogs bowl and refilled water. Tidied up clients room and the ants in her bedside drawer as she hide chicken in there. Took off the comforter 'coz of the ants too and washed it. Emptied her commode and cleaned restroom.Left before 1:30 pm as she was chatty.
