Jan. 27 (Monday) = Turned on dryer and laundry as soon as I got in. Medicine for yesterday (hydrochlorothiazide and Guafenesin and Atenolol) was untouched.Client awake. Back pain. Says she took pain medicine last night when asked what time she took it. Then says what she took this early morning was the green med. (which is for the itch.) Asked her again and says she took white meds. early morning (explained it again to her the white ones are or pain and the green ones are for the itch.) Says he took the white ones last night. Wants muscle rub on her back so put muscle rub. Then says her back is burning. Used towel on back. Sponge bath. Changed dress. Had tea and a waffle, took her hydrochlorothiazide and had 1 Ibufrofin and half of vicodin for pain at 10:30 am. Swept bedroom area. Emptied commode. Told client RN, Mary is coming today to visit. Client don't want it and asked me to call RN not to come. Had a voicemail from RN about her visit. Called RN back and told her client doesn't want the visit today. Asked about client condition, told her client was in pain when I came in and just wanted to rest as she was awake most time last night. Says she will just come on Wednesday instead. Told it to client, but still client don't want the visit. Tidied up kitchen area.Defrosted 1 chicken leg for lunch. Changed bedsheet. Changed towel rug. Tv turned on. Client fall asleep. Prepared lunch. Client had lunch passed 12 noon. Had grilled chicken leg with ketchup, half of baked potato and sweet corn and water for drink. Liked the chicken. Had Atenolol. 4 halves of vicodin (2 tablets) prepared for later and put on client's bedside table with 2 hydroxyzine (remained from yesterday) on another bowl. Told client she could take the pain med. 4:30 pm. Call the time to check first and meds. are every 6 hours. Checked phone: working. SAys she have to wait for her check before paying my office. Changed bedroom's light bulb before leaving. Left 1:15 'sh pm.
Jan. 28 (Tuesday) = Continued laundry and dryer as soon as I got in. Client awake, sitting in bed. Says hadn't taken pain meds this morning, but took them last night. Client had tea and fried egg. Had 1 Ibufrofin and a half of vicodin 9:30 am. Tidied up bedroom. Turned on tv. Client fall asleep. Cleaned kitchen floor, tidied up kitchen. folded clothes. Changed clients dress. Client had lunch passed 11:30 am. Had chicken sandwich on Hallah bread, with tomatoes and pickles and water for drink. Says dog had some of her lunch. Still saying about the pain and asking if she could take more meds. Told her not time to take more meds. Swept bedroom carpet. Watered plants on the front door. Got Scripps mail that says put on the Scripps folder until clinician ready to fill it up (put it in Scripps folder.) Client asleep after lunch. Had her Atenolol med., about 12:35 'sh pm. Cut toe nails. Brought date nuts to her room and explained her pain meds (4 halves of vicodin and 1 Ibufrofin tablet.) Asked for the green pill so put 3 of those too on the bedside and explained again it's for the itch and white pills for the pain. Says she is itching when asked so she took 1 hydroxyzine just before 1 pm. Told her she could take pain meds at 3:30 pm. Left 1 pm.
Got call from Scripps home care case manager Sandy, asking about RN, Mary's visit ad client's condition on my way to the office.
Jan 29 (Wednesday) = Dog outside on top stair, coughing. Client awake. Back pain still. Says took meds last night by 6 pm. Had tea and waffle. Had her hydrochlorothiazide and Ibufrofin and half vicodin about 9:30 am. Hot towel on back ad had sponge bath. Turned on tv. Did more laundry. Changed client's dress. Changed bed sheet. Client thinking of Marie Callender's for lunch. Client had fallen asleep 10:30 'sh am. Scripps RN, Mary says she will be visiting between 11 am to 12 pm. Prepared grilled chicken. Client had lunch passed 12 pm, had grilled chicken leg and thigh, (cut it in bite sizes for her), steamed zucchini, some Nacho chips and water for drink. RN arrived passe 12:30 pm. Vital sings ok. Talked with me about client's supposed to be "s." But to nurse, it's just wasn't "s," just cant be asclient hadn't been outside, but more likely flea bites. Tried to call moile physician, called their office, told me P.A.Jimmie Thompson will call her. Says she told them if maybe he thinks it's flea bites as the skin scrapping wasnt done. RN left passed 1 pm. Client had Atenolol and Guafenesin. Told her the refill ready from Rite-Aid pharmacy (Ibufrofin.) Left 1:10'sh pm to pick up refill. Paid with check. Prepared 12 Ibufrofin tablets for client, also about 4 hyroxyzine as well, with note for both on what it is for and how often to take, with tomorrow's other meds on kitchen table. Refilled hydroxyzine. Left about 1:30 pm.
Jan 30 (Thursday) = OFF!
Jan 31 ( Friday) = Dog barking, lease stocked on door and hadn't been out yet. Client awake, lying in bed. Says about her son'scall last night, about mobile physician's office called about her vital signs ok, but a bit high on cholesterol. Back pain. Says took meds. and feeling nauseated. Client had tea, took her hydrochlorothiazide and 1 tablet of vicodin, then had a waffle while I put muscle cream, and used warm towel on her back after as she asked. Turned on tv. Washed soaked towel rugs. Continued comforting and using warm towel on client's back for about an hour. Made list for Vons. Called Marie Callender's for lunch menu as client thinking of it. Client spoke to Carloson her bank to check her balance, says she have over 1K in her checking. Wrote check for Vons. Decided to skip Marie Callender's for today. Left 11 am for Vons. Gor items and called client for lunch $5 Friday they have. Decided on Burger King instead. Paid groceries by check. At BK next and got double cheese burger ad value fries. Spent $3.22 with my card. Arrivd back at client 12 'sh pm, warmed up fries and mae steamedzucchini and broccoli. Client ha lunch passed 12 noon. Had double cheese burger, steamed zucchini and broccoli, fries and Diet Coke for drin. Only had half of the sandwich but ate the slice of Boston cream pie. Says she will finish sandwich and the est later. Client sleep. Had her Atenolol before 1 pm. Says sandwich wasn't that good. Prepared 2 tablets (cut in halves) of vicodin set on her bedside table. Told client she could take vicodin by 3 pm as she took 1 9 am. Left passed 1 pm.
Feb. 01 (Saturday) = Client lying in bed, awake. Back pain. Client had cocoa and fried egg while im using warm towel on her back. Then asked for muscle cream and says it burns. Used cold towel. Took her hydrochlorothiazide and took 1 ibufrofin and half of vicodin at 10:11 am. Turned on tv. Changed bedsheet, and changed clients dress. Swept bedroom and emptied commode. Laundry continued. Changed towel rugs. Tidied up kitchen. Prepared lunch and tomorrow's meal. Made sliced hotdog, fried with sweet corn and steamed broccoli. Grilled salmon fillet. Client had lunch a little passed 12 noon: had 2 salmon fillets, baked beans and steamed zucchini and diet coke for drink. And served her small slice of boston cream cake. Client ate all, and likes the fish. Prepared meds. Client had atenolol, 1 pm and explained her how often the ibufrofin could be taken, every 8 hours. Put it on bedside table. Left passed 1 pm.

Jan. 20 - Jan. 26
Jan. 20 (Monday) = Dog outside. Client awake. Didn't heard complain about back pain like she usually does, but complains of itching. Rubbed back and used hot water. Hadn't touched pain med. Had tea and a waffle and took Hydrochlorothiazide and antibiotic before 10 am. Sponged bath, changed clients gown and changed bed sheets too. Client fall asleep, tv turned on. Tidied up kitchen. Laundry. Texted her son about mobile physician coming but didn't know time, and told client's son about me back on my other job, 2 - 10 pm, told him to call office but says says things about Merrillee talking to him about overtime but how come I have 8 hours with other patient and 4 hours with his mom, makes it 12 hours. Told him its two different patients. Still wants me to STAY until mobile physician arrives. Client had 1 vicodin tab about 11:30. Left passed 11:35 am to get lunch @ Burger King, 2 original chicken sandwich with coupon, value fries and sundae pie. Paid $8.+ with my card. Client had lunch passed 12 noon. Had an original chicken sandwich, value fries and water for drink. Mobile physician arrived before 1 pm. Checked on client. Talked to me after that it wasn't shingles like what client had before, more of like scabies and it is contageous specially with weak immune system. But told me not to tell client and told me to let him know if I ......... Called Rite-Aid pharmacy twice t ocheck for prescription but not yet. Gave client Atenolol 1:30 pm before leaving. Doesn't want to pay office yet, says will have to wait for check. Reminded her she's already 3 weeks behind. Left passed 1:35 pm.
Jan. 21 (Tuesday) = Dog was outside. Called Discount Pest Control about rescheduling their service at client. Also called Rite-Aid pharmacy and asked if there's any ready new prescription, talked to the Asian, male pharmacist and says 4 prescription is ready to be pick up. Client awake and itches and told her I need to pick up her meds. Client had tea and had taken Hydrochlorothiazide 9:30 am. Client wrote check for Rite-Aid and asked for tangerine nail polish, soda pop, and spoke to Claudia for a cream for hand for dead skin. Picked up meds, consulted with the male, Asian pharmacist. One med wasn't covered with client's insurance and paid $25 for it, total spent $55+ with client's check: Got Diet Coke, the tangerine nail polish and hand cream, peeler. Arrived back and client had 1 muffin before med. Took green med for her itch 10 am and the Guafenesin and put cream (the three times / day cream) and assisted on changing her gown. Had changed bed sheets and the linens. Tidied up kitchen. Spoke to Merrilee about client condition, about my concern of "it." Was told to wear gloves. Laundry and dryer. Client fall asleep. Had texted her son about it. Woke up client to let her know I'm going get lunch and that I would pay for it first since she said,"where' are you going to get the money from?" Went to Popeyes and got leg and thigh and biscuit, and paid $3+ with my card. Dropped by to Rite-Aid and got gloves as I had ran out of gloves. Cost $8.99. Got back and warmed up food. Client had lunch around 12 noon. Had Atenolol taken. More cream at her back. Had shampooed vacant room carpet and changed bed sheet in there. Was washing back door stairs when received call from client's son. Had told client at 4 pm she have to take the green med. and that it is every 6 hours. Also asked her about vicodin, and says she took 2halves (1 tablet) early morning, maybe 8 or around 8 am. Left already passed 1:30 pm. Had called client 4 pm, reminded her of taking med.

Tidied up client's bedroom. Wants to call hospital and ask wjst to dp with her back pain, told her RN Mary of Scropps home care services is coming at 11 am. Client got upset and doesn't want visitors or anyone coming, saus sh e jad a bad might and in pain amd jist wamted to rest, askede to call Mary amd tell her mot to come today. Called Mary and told her situation. Says shw will just come on Friday instead. Put mudcle rib pn client's back, them used cold towel as sje said its hot now. Changed client's dress, turned on tv. Decided on Subway for lunch. Laundy continued and dryer. Wsnted gootlong hotdog and wheat bun to be returned on Smart and Final, wrote check for dog f food and pther items she wants. Left about 11 or passed 11 am. Got taffy candy, nachos, paper towel, soup. Marie Callender's tv dinner (rice and batterered fish) and dog dog food. Returned items, bums is accepted, credit given but footlomg hotdog wasn't accepted. Paid with client's check. At Subway next and got tuna sandwich. Paid $5.40 with my card. Back around 11:30 am. Warmed up lunch. Client had half of the tuna sandwich and water for drink. Fall asleep. Had Atenolol a little after 1 pm. Hot towel on back, and then the itch cream (the 3x/day cream,) explained meds (pain med: vicodin) and green itch med hydroxyzine to client) wrote note on what each meds is for and how often to take it. Also told her both the am and pm meds in the kitchen and the meals prepared. Checked phone is working properly and told client home care's office is pasted on wall and she could call case manager anytime if she needs help someone to be there. Left 1:35 'sh pm.
Texted client's son around 4 pm (?) on client's condition, and been doing it the passed weeks. Son called me and spoke on client's condition, about Scripps, Sandy, and what she said about his mom's condition, and him concerned about client's being alone tomorrow since I'm off and asked me if I'd be able to come. I said I already have plans and says she would have to call Merrilee for it.
Jan 23 (Thursday) = OFF!
Jan. 24 (Friday) = Client awake, lying in bed. Says she's in a bad condition: back hurts and awake most time of the night. Meds. bowls were empty on the bed. Had 1 vicodin and 1 Ibufrofin around 9:15 'sh am. Doesn't want anything to eat for the meantime. Tidied up bedroom area. Washed dishes, tidied up kitchen area. Continued laundry and dryer. Got a voicemail from Mary, Scripps home care RN, saying she'll be there between 12 - 12:30 pm. returned call. Says she'll be coming on Monday instead as she have patients that called, she needs to see. Swept client's bedroom area, emptied commode. Made client a waffle, which she ate. Had the hydroxyzine (green tablet) about 10 am. Put itch cream on back. Back rubbed with hair brush. Changed dress. Changed bed sheet. Got call from client's son,asking updates on client, if RN, Mary's coming, told him RN will be coming on Friday, clients condition, him talking to Merrilee about adding more hours/shifts if needed, learned client behind her payment for office for 1 month now, and says he called his mom last night and was told by client I was there/worked yesterday so he thought I went to work yesterday, asked if client was confused. Decided on having the hot dog or lunch. Folded dry clothes and bedsheets. Client fall asleep. Had prepared lunch. Client had footlong hotdog sandwich on Hallah bread, steamed spinach and water for drink. Prepared clients pain meds (5 tablets vicodin cut in halves) and hydroxyzine, 5 tablets. Fall asleep after lunch. Showed client the mails: 3 from Scripps, and 1 bill from blood draw from mobile physician, $213. Since I'm going to the office, asked client if she wanted to pay for the office as she was 1 month behind. Says she's been thinking of it but doesn't want to get money from her savings, asked me the date today and says she have to wait for her check. Says she will take care of it on Monday, hopefully. Client took Guafenesin 1 pm, keeps on dropping atenolol on the bed. Hadn't taken it. Explained her the vicodin and hydroxyzine meds., time she could take it, every 6 hours as needed. Let her know there's soups and meal for her meal. Left passed 1 pm.

Had Ibufrofin and half of vicodin 11 am for pain. Left passed 11 am. Got taffy, dog food, paper towel, Diet Coke 1 tv dinner and called client for the lunch meat, decided on corned beef. Also got trail mix and dates with nuts. Paid by check. Had checked items and realized I forgot the corned beef so went back inside to get it. Spent $6+ with my card. Back passed 12 pm. Prepared lunch. Client had corned beef sandwich on Hallah bread, steamed spinach and Diet Coke for drink. Says corned beef wasn't as good as Akins. Prepared meals for later and tomorrow. Prepared steak and sweet corn, sliced grilled hot dog and baked potato, corned beef sandwich, Had Atenolol passed 1 pm. January 13 - January 19
Jan. 13 (Monday) = Door's locked. Client awake, calling me. Hadn't complain much of pain like the past days when as long as I got in she says back hurts and I would start day with massage, rub, and the hot towel. Says she hadn't had much sleep last night, got little sleep only. And also says she owes Noelia $5.50 for Chinese food last Saturday. Client had waffle and tea before or around 10 am and had took Hydrochlorothiazide after. Put pain cream on client's back. Tidied up room, kitchen and continued dryer. Was telling about a Windmill recipe on the ad, but I can't find the ad, not on kitchen table, not on trash, and not on client's walker as she said. Says a recipe about a chicken and we could get the chicken from Smart and Final. Then asked her what she wants for lunch, and says we could set aside the chicken recipe from the ad and I could go to Akins for lunch. Put cream on back and massaged. Had asked client what time she took pain med, says she hadn't take any. Then says she wasn't sure if she had taken a pain med., maybe morning. Client wrote check to be cashed in to bank, $30. I called DZ Akins and put in usual order, talked to the Mexican man I've also talked to for order often. Says I'd pick it up before 11 am. I left passed 11 am for bank, Carlos: teller. Had cashed in the $30. At Akins next and picked up order, felt the bag was cold so I checked on the items. Bread and 1/2 lbs. corned beef was cold eventhou I ordered warm. But only told the guy who handed me the picked up order that I ordered hot 1/2 lbs. corned beef. Lady employee changed it. Paid with client's cash. Dropped by to Ralphs on my way to get dog foods. Paid with cash as well. Got back, and prepared lunch, made corned beef sandwich and zucchini, steamed. Served lunch 11:47 'sh am, the sandwich, steamed zucchini and water for drink. Also returned change,$13.27. Checked on client passed 12:20 'sh pm to give her pain medicine, says she just took it (Ibufrofin) and I gave her 1/2 of the Vicodin, told her it's her other pain medicine. Client in pain. Went on and used pain cream, client asked for hand Body cream too to be tried on her back, which we did. Also used hot towel as she had suggested. Client says she will pay me first for what she owes me for lunches, from BK and Subway. Paid me $12. (Owes me $6.40 and $6.88) but only told her $12., so she paid me $12. Got a missed call from her son, about getting client into Scripps Hospital, and concerned about client's coughing, as he remembered that how client's husband's pneumonia sits in. Only left voicemail about client having shower when she called me so hadn't noticed cp. Continued shampooing vacant room's carpet. Client thinking about calling Ralphs for coffee cakes and Vons for jello. Told her I'd change her bedsheets first. Tidied up client's room, swept carpet, and changed bed sheets and almost done when client walks inside. Says back pain, and lied down in the bed. Emptied commode and soaked soiled bedsheets and washed soaked clothes. Client decided Popeyes for lunch, Tuesdays special, leg and thigh. Says she have to pay Noelia for the $5.50 she owes her for Chinese food. Client gathered her coins for lunch, got over $4. Turned on tv before going, and client asking if she could take another Ibufrofin, told her not, as she already took 2 as she said when I came in, says ok, she wouldn't. Left 11:50 'sh am. Got leg and thigh chicken, mild, and decided to add a biscuit. Got back passed 12 noon. Warmed up thigh and biscuit and made steamed spinach. Woke up client for lunch around 12:20 'sh pm, chicken thigh, steamed spinach and the biscuit and water for lunch. Made baked potato to add to the chicken leg I had set aside for client's meal later. Called Rite-Aid pharmacist and told her about getting in at 9, and being told by client she took Ibufrofin, 2 tablets of 600 mg. And asked if it's ok for client to have another one again as she client wanted to take another one before I go to get lunch. Pharmacist says two 600 mg, taken together would be 1200 and that could cause liver damage. Ad client should increase hours of taking med again, like wait 8 to 10 hours, don't take any ibufrofin pretty soon, maybe late tonight or tomorrow but vicodin can be taken. So walk into client's room, was sound asleep, woke her up to give 1/2 of vicodin and I was told she took another Ibufrofin after I left to get lunch. Called Rite-Aid pharmacist again, this time talked to a male pharmacist and told him story again, and added that I just went to clients room to give her 1/2 of vicodin and client told me she took another Ibufrofin when I went to get lunch. Thought girl had her Smart & Final check and afraid that girl could cashed in money, or used her check. I found the Smart & Final check, (dated yesterday (Jan. 16) but client say it was written poorly, she could have written another check. Worried. So I told her I'd call Merrilee and asked about it. Called Merrillee, and asked, said Karla, (the other girl) didn't went to store yesterday at all, but the other check client wrote for her was to pay Karla $8 she spent to pick up client's prescription. Told it to client, that the check that I found on the floor in her bedroom was indeed the check she wrote for Smart & Final, and the another check she said she wrote was for payment, for Karla when she paid for picking up client's prescription. Client wrote 2 checks: for Rite-Aid and Smart & Final . Got call from Mary, Scripps, RN homecare services. Asked me until what time I'm at client, told her until 1 pm, so she said she'd be there 12 noon. Client had Azithromycin (antibiotic on the fridge) 2ndday today, before 11 am. Left for stores 10:50 'sh am. At Smart & Final first, got items, called client for Hebrew National footlong hotdog's price, $9.99, client said it's ok so I got it (10 pcs. in a package.) Paid with client's check. At Rite-Aid next and got over the counter Guafenesin 5R 600 mg, and hydrogen peroxide. Paid with client's check. Got back passed 11:30 am, client watching tv. Prepared client for Scripps RN's visit at 12 noon. Client at commode and tried to get grab onto her walker after as I keep on telling her it is better in the living room to go. Tired and says all she wanted was to sleep. So told client if she feels that way that she can't go to the living room, she could just stay in her bedroom. Client fall asleep. Preparing soup and boiling 2 ft long hotdogs, accidentally sent an unfinished text to client's son about what meds. client had taken. Son called, talked to me about yesterday and office, Merrille, the meds. etc. the antibiotic that client took 2 tablets on the first day. Client had a cup of mushroom soup, consumed almost half. Called Merrilee and told her, she called Karla and confirmed. Then called me back and says she talked to the pharmacist and it was 2 tablets on the first day, together. Told her also about the Scripps RN coming that client said. Scripps homecare service, RN, Mary R. arrived passed 12:40 'sh pm. Got client's medical history, and asked client some questions. Got client's vital signs, temperature, 97, BP, 123 (or is it 120?) / 57. Client had hotdog sandwich, only consumed half of it, says hotdog was hard, buns not good. Client had Guafenesin tablet and Atenolol 2 pm. (Left 1 Atenolol) at kitchen for later with a note, "tonight. Got a text from Merrilee, "how did it went?" and I replied "Not done yet, RN arrived before about 12:45 pm, but still here." Was about to text son about me having to leave, already 2:33 pm as got a call from my mom asking where I am, I'm using her car. I got my car fixed BUT brother used my ca, and mom knows I'd be home before she lives. So mom had to call in she'd be late 30 minutes. Son called me just when I'm about to text him, asking about updates how it went, told him RN, Mary's still here, so he spoke to her. RN left passed 2:35 'sh pm.Put half of sandwich back on fridge and put the halves of the vicodin (3 tablets) on bowl, on bedside and explained it to client, time of taking, 4 pm and 10 pm for the night.) Got graham fudge, Greek salad and can of sweet corn, and UT newspaper. Back 11:45 'sh am. Continued preparing the meals. Checked on client as hear her kinda growling, going back to lie down in bedfrom commode, scratched back. Made steak for lunch. Client had steak and Greek salad and water for drink for lunch passed 12 noon. Prepared meds. 3 tablets of Guafenesin good until tomorrow, Sunday until I come in Monday. 7 halves of vicodin. BP mes on kitchen table as usual. ALL with notes on how often meds are taken. Awakened client 12:50 'sh pm and gave her Atenolol and explained meds and put vicodin and Guafenesin 3 tablets on separate bowls with the notes. BP mes on kitchen table as usual. ALL with notes on how often meds are taken. Awakened client 12:50 'sh pm and gave her Atenolol and explained meds and put vicodin and Guafenesin 3 tablets on separate bowls with the notes. Client wrote check to pay Noelia for the Chinese food last week's Saturday, $5.50. I got the check, told client I will ask office for Noelia to have it. Left 1:10 'sh pm.
NOTE: two halves of vicodin (1 tablet) left by client on bowl along with taffy candy wrappers. Texted Noelia about the check from client, say hold onto it 'till we met again.

Jan. 06 - Jan. 12
Jan. 06 (Monday) = Dog outside. Client awake, lying in bed. Tells me her sons visit. Says it was a good visit. Then feels pain. Massaged back and used warm compress. Client had tea, took Hydrochlorothiazide. Tidied up kitchen area. Laundry and dryer continued.Turned on tv. Opened gift with client her son and family gave me. :) Decided on Burger King. Asked her if she want to try Alaskan fish sandwich, and says yes. But client says she doesn't have enough money and was about to find coins again. And asked if I have money with me, she would just give it back tomorrow. So I said I could just pay for it first. Left maybe almost 11:20 'sh am. Got Burger King's Alaskan fish sandwich, chocolate pie, and value fries w/o salt. Spent $6.88 with my card. Client had it for lunch with water for drink. Client fall asleep after lunch. Had pain med. 1 pm. Explained to her time of taking the next tablets and understood. Massaged back again with pain cream. Wrote check for office and reminded her she still owes office for 2 weeks. Left a little passed 1 pm.
Jan. 07 (Tuesday) = Dog outside. Client already awake, lying in bed. Then the pain and back itch. So scratched back, massaged with pain cream, back rub. Made tea. And client fall asleep when I got back so had just let her asleep. Client was deeply asleep. Tidied up kitchen. Did more laundry and dryer. Tried to wake up client 10:30 am for grocery list, but client deeply asleep. Let her slept more. Stayed in room for a while, and spoke to client when she made movement. Client woke up 10:40 'sh am. Says back started hurting again. Massaged with pain cream, warm water, back scratches. Says back's hurting and itches. Client wrote check for Ralphs. Turned on tv. I left 11:40 'sh am already. Got items from Ralphs, paid with the check. Client sleeping when got back.Prepared lunch,beef bacon sandwich. Client had it for lunch, with steamed spinach and hot cocoa for drink. Woke up client for lunch. Wants to take pain med.before lunch but told her she have to eat first. Says she took pain med.before I came in but don't remember the time when asked what time she had taken it. Prepared Healthy Choice salmon and grains tv dinner. Client lying in bed after dinner, sleeping but was awakened when I came in to pick up plate. Told her she can now take pain med, its about 1:30 pm. Says she had already taken it before lunch since she was hurting. Rubbed her back and put pain cream. Refilled pain med. Client asked if I want to stay an hour more or 30 minutes, I said I could only stay 30 minutes. Says ok, I could use it to rest since it's late and she knows I'd be running around so I said ok. Went back to kitchen and finishes things needs to be done, washed dishes. Then massaged back again with the cream, rubbed, dialed phone for client for her as she want to talk to him about Pest Control guy. Told client time of taking her next doses of pain med. Left 2:40 'sh pm already.
Jan. 08 (Wednesday) = Dog outside. Client awake. Says back pain. So spent almost an hour massaging her back with the pain cream, back rub, warm compress. Says took pain med. early but don't know remember her time of taking it. Turned on tv. Reminded client about Chase bank, money needed to be deposited, $500. Made tea and boiled egg. Client had the tea while I massaged her back again, pain cream, back scratched, (says back itches and hurts) warm compress. Soaked soiled bedsheets, dried clothes. Tidied up kitchen. Prepared meds .for tomorrow. Client made check for $500 for Chase bank and called Citibank to check her balance on checking: spoke to Carlos, says she was told she have/had $700 on her checking. Was about to look for $2 coins so I told client I could just pay for lunch first, (it's already passed 11 am and have to go to the bank.) At Citibank and cashed in $500, teller: Julian. Deposited $500 to Chase bank, teller: Katara. went to Subway and bought oven roasted chicken, with Sunny chips. Spent $6.40 with my card at Subway for oven roasted chicken. At Rite-Aid next and got writing tablet, pain cream, a Hershey bar, paid with client's check. Back passed 11:35 'sh am. Prepare lunch. Clint had lunch before 12 noon, half of the sandwich, some chips, and hot cocoa for drink. Client says she took 1 pain med. about 30 minutes ago (makes it before 11:30 am.) Reminded her, pain med. SHOULD be every 8 hours. Client says, she was hurting so she took pain med. Had made beef bacon sandwich and half of oven roasted chicken on fridge. Client took her Atenolol before 1:30 pm. Explained her times of taking pain meds. and had refilled pain meds too. Massaged back again, back rub,backs scratched. Wants me to come tomorrow instead coming on Friday.
NOTE: Client spoke to CVS pharmacy and asked again about osteoporosis and uses of pain cream and hot water. Then called Alvarado too.

Client called Akins for soup, learned it was almost $10 and decided not to. Called Ralphs next she decided for Ralphs soup instead, was told Home Style soup was on sale. Client looked for change. Not enough so wrote check instead. Called her son to find the doctor she wants, the one no longer with the mobile physician. Left a little passed 11:30 for Ralphs. Got items. Paid with client's check. Heated soup as client says will only have soup. Client had soup passed 12 noon. Took pain med. 1:30 pm (says want to take it now as she was hurting.) Says she took one in the morning, around 6' sh am. Mae beef bacon sandwich. Back rubbed and massaged again. Client wants to be called tomorrow morning when I told her "see you Saturday," as I'm off tomorrow, she exchanged Thursday to Friday yesterday. Left passed 1:30 pm.
Client had missed call. Called client back. Says just her idea, she was feeling better, not well but a bit better and scared of being alone and wants me to come. Told her I could come but could only stay until 11 pm, I have to pick up mom at 12 am, I used her car to picked up dad and drove him to his doctor's appointment and now have to pick up mom 12 am. Had talked to client instead for 2 hours. Reminded her of her wanting me to call her tomorrow, but then started thinking of me coming and decided on what to have for lunch.
Jan. 10 (Friday) = Was supposed to be off today, but had talked to client last night and wanted me to give her a call today and see if she wants me to come. Called client but phone's off hook. Texted her son (with client prolly needs someone at night too.) And called Merriliee.
Kitchen's phone and bedroom was off hook. Client awake. Says back pain. Spent time massaging back with pain cream, back rub, hot water. Client had tea and took Hydrochlorothiazide. Laundry and dryer. Tidied up kitchen. Client wrote check for Vons. Son called about talking to client to go to mobile physician's office and get treated. Talked to client while massaging her back again. Doesn't want to. Called son, left a voicemail. Got a call from son and says plan B is for the mobile physician to come (PA) with an RN and wrap client. Also wrote for check for Rite-Aid for phone cord. Got call from client's on at Rite-Aid saying he'll call mobile physician and confirm time they will arrive and will call me back. Got phone cord and another muscle rub. Paid with client's check. At Vons next and got items needed. Paid with client's check. Just got back to client's place and got voicemail of hers on about mobile physician on their way in 25 to 20 minutes. Prepared client. Walked client to the living room. Client was in pain and exhausted. PA and RN arrived, took client's vital signs, says all are good. Wrapped client and had her took the breath test on a tube. Prescribed Vicodin and says take 1/2 tablet as a start. Can take it with Ibufrofin. PA and RN left and client gave them the strawberry shortcake. Client asking if I could stay overnight. Told he maybe until 11 pm. Walked client back to her room. Prepared lunch, 1:45 'sh pm. Called her son about the visit and client's asking me to stay overnight, says "that's up to you Precy." Client called me about her wrap, asking if she could take it off and showed wrap to me. Took wrap off client as it got all wrinkled up and got into her skin. Client was upset of her son and PA. Continued preparing lunch (fried breaded salmon.) Client had zucchini, half of baked potato and salmon for lunch and water for drink. Called Merrilee for client wanting me to stay overnight and I said she have prescription too,and maybe i it will work I could just stay until 11 pm. She asked me to call client's son and tell him to give her a call. Son says he's busy overlooking important event, asked Merrilee to call instead. Called Merrilee back and told her, give her client's son's phone number. Left for Rite-Aid for prescription. Received call from Merrilee,asked me until what hour I've been there, says 9 am and if client wants me to stay overnight, I will be thee tomorrow too , and that's until 1 pm. Says she will send someone else. Texted client's son and let him know. Got a call from him while on Rite-Aid. Picked up Vicodin prescription about 4:10 'sh pm. Asked lady pharmacist too about taking Ibufrofin and Vicodin. Back before or 4:30 'sh pm. Changed towel rug on her room and started shampooing vacant room. 4:45 'sh pm, call from Noelia, reliever. Talked to her about routines. Introduced her to client. Client's ok with her and nice. Muscle rub at client, and hot towel at maybe 5:30 'sh pm. Left 5:45 'sh pm already.
Jan. 11 (Saturday) = Texted Merrilee 4:20 'sh am about me having cold and if she could send someone tomorrow for the client. Says she will call Noelia 7 or 8 'sh am and will let me know. Got a call from her 7 'sh am, and says Noelia is ok with it. But asked me to go to client 9 am just to make sure Noelia can get inside.
At client's place 9 am, then Noelia arrived and Merrilee. Showed her client's meds., showed up to client and just said hi, that I'm sick and won't be able to work and client was told to call office when she needs someone. Emptied commode and rinse. Wrote down client fall Dec. 20 on Merrilee's piece of band paper. Also told her client fall Dec. 20, got the back pain, Dec. 31 and received x-ray result of broken rib Jan. 4.

December 30 - January 05
Dec. 30 (Monday) = OFF (Since I went work yesterday, Sunday. Had exchanged it. Sunday, son expected to visit but postponed it or next week.
Dec. 31 (Tuesday) - Jan. 01 (Wednesday) = Dog was outside. Client already awake. Says dog wasn't good last night and arthritis pain. Says she had back pain yesterday and still have it.Had fall asleep after having tea. Tidied up kitchen. Massage back, shoulders and back with pain cream. Reminded client to call her bank and check her balance to make sure she have money for tomorrow's grocery as bank close tomorrow, January 1, for New Year's Day. Client did and spoke to a lady employee. Says still have $23/24. Wrote check for Rite-Aid for pain cream, Ginger Ale, chocolate bar. Gave me $6+ all sort of coins for Burger King, lunch. Left around 11:30'sh am, went to BK and got original chicken sandwich and value fries w/o salt. Spent $6.03 with client's $. At Rite-Aid next and got items. Paid with check. Client had lunch, orig. chicken sandwich, fries, and Ginger Ale for drink. Massage pain cream (got more from Rite-Aid as we just cut an almost empty cream.) Warmed up fries again as it got cold. Client took Atenolol before 1 pm. Wanted to call son to call mobile doctor so I dialed number but phone's busy. Client says she will be fine. Had put more pain reliever on bedside table. Wrote down her son and my phone number on paper and told client to press hard on phone pad to get through. Left 1:15 'sh pm.
Night around 9 pm, missed call from client. Called client a little passed 10 pm, asked me if I could spend night in there, sh's in pain, could hear dog barking in the background. Told her I'd asked a friend first to either drive me, or drive me to Walmart so I get mom's car and drive it instead. Told her I'd call her back. Called her son, then called agency and ha spoke to Merrillee. Told her son doesn't want client to get upset with it, that was I was told last time when I stayed i there overnight but son knows thou I'd spend night in there. Says flat rate, I said ok. :) Called client back and said, I'd come.
Arrived at client, almost 11 pm. Client lying in bed, in pain. Did massage back, ad shoulders with pain cream. Turned on tv. Chit-chatted. Said hadn't had dinner (she didn't touched the baked potato, and corn I had prepared for her dinner.( Client had boiled egg and corn at 12 midnight. :) Looked for pain reliever I left but cannot find it, seemed to be dropped on the floor. Clint described pain as crashing pain. Massaged back everytime she feels the pain. Says she can't find 2 rings I found before (I think the rings I got from Kmart.) I looked for it around 1 am. Asked beads to be hanged on the small house-shaped drawer, and I did hang them as se wanted while client looks. Client fall asleep before 2 am. Awakened by pain a bit passed 2 am, so I massage an rub her back until she falls back to sleep. Client at commode almost 4:30 am. Asleep again 5 'sh am, but awakened by the pain everytime she turns or repositions herself. Back massage/ rub with the pain cream until she fall asleep again, before 6 am. Anted me to asleep and rest since she's wondering how come I'm awake. Client awake, chit chatting with me and thinking of grocery lit for Smart and Final for the morning, as she wants pancake or maybe waffles. Doesn't want tea. Tv on. Wrote check for Smart and Final and Rite-Aid. Asked client again her balance, now says she thought its $33, so told her she said about $23 yesterday. Texted her son and told him how client was for the night. Left for Smart and Final passed 7:30 am. Got items and paid with client's check. At Rite-Aid next, got pain cream, Spearmint gum, a soup, paid with client's check. Back at client passed 8 am, in pain again, massage back with pain cream and put 1 Salonpas patch on the center of the back. Client watching Rose Parade. Told client I forgot pain reliever, but will just prepare breakfast and get back to Rite-Aid for it. Client had tea and a waffle, then had Hydrochlorothiazide after, before 10 am. Asked client for another check for pain reliever. And she did wrote one. Also wanted me to look at CoverGirl night cream, says she saw it on tv ad. Left for Rite-Aid, got Tylenol pain reliever (told client I'd get it instead of the Rite-Aid brand, Acetameophine she's been using,and agreed). Took 2 Tylenol 10:28 am. Fall asleep, but would be awakened by pain. And I would massage back everytime. Also used warm compress and seemed to help. Assisted client on changing dress. Wants El Pollo Loco but do't have money. Idea of calling her son to lend her money for lunch. Told her I'd go and pay for it first instead. Left for El Pollo Loco about 11:14'sh am, got 2 combo, leg and thigh, cole slaw, rice, and 2 churros. Paid with my card $8.40. Got back passed 11:35 'sh am, client lying in bed, with lipstick. Prepared lunch. Client had the chicken thigh, rice, cole slaw and 1 churros with warmed up water for drink. Made chicken flavored rice-a-roni rice and steamed spinach for later meal. Phone rang, client called me and says granddaughter on the line ,wanted to greet me Happy New Year, got on client's room but lost her on the line. Called granddaughter back, spoke to er and greeted me. :) Then passed phone to her dad. Client fall asleep soundly and had let him know, also let me know mobile physician will arrive between 11 or 11:30 am tomorrow. Awakened client for Atenolol but was too sleepy, so just told her I'd put Atenolol on the medicine box on her table. And that pain reliever, Tylenol was on the big green bottle and explained the times she can take them, ONLY one Tylenol this time for every 6 hours. And she could only take total of 4 more Tylenols. Client agreed. Let her know I'm leaving at a little past 1 pm. Client too sleepy, but wanted me to rest first for an hour before going as I had't had sleep. Agreed, but told her I'd just g and won't wake her up, client nodded. Texted her son and told him I'm gonna go, and that client is still sleeping and reminded her time that she could take more Tylenol. Left 1:18 pm.
Jan. 02 (Thursday) = Dog outside. Client sleeping. But says about the pain when awake.Client had her tea and a waffle. Took Hydrochlorothiazide after, around 9:40 'sh am. Massage her back with pain cream and also used warm compress. Turned on tv and shampooed vacant room. Tidied up kitchen area. Client says a guy called, the tech., or for her x-ray tomorrow. Guy says arriving passed 1, but client says I leave at 1 pm and says the guy says he'd be here between 11 to 11:30 am instead. Laundry done. Wants to try and have shower since mobile physician is coming today. Prepared shower. Client had shortness of breath when walked from bedroom to vacant room, sat for a few minutes and tried to et into the shower but decided not to force herself. Mobile physician arrived a little passed 11:30 am. Checked client's BP, O2, and back. PA ordered x-ray, and says tech will call before coming, prolly tomorrow but will call us first. Client declined a stronger pain medicine prescription. Client gave 1 clown doll on PA's MA which was half Filipino. Mobile physician just left, and client asked for the prescription, went after PA which was just pulling off the front yard and got the prescription for client ( Ibufrofen 600mg/8 hours). Prepared lunch for client: Marie Callender's battered fish (client had with BK ketchup) and rice and water for drink. Set aside one fish and put it together with rice-a-roni rice on the fridge and steamed spinach which was readied for client. Called her son and let him know about the visit. Client walked back in her room. Left for Rite-Aid 1:15 'sh pm. Waited 15 minutes for prescription. Paid with client's check together with a small denture glue and 1 more pain cream. Back before 2 pm. Client took 1 Ibufrofen 600 mg. Warmed up chicken noodle soup for client for later and told her about it, together with the meal for dinner (rice-a-roni, with steamed spinach, and the other Marie Callender's battered fish.) Reminded her the Pest Control bill. Had 5 Ibufrofen tablets ready on her bedside table and explained client it is every 8 hours, that she have to have dinner, or eat efore taking more of it, next med. would be @ 10 pm and next on 6 am. Told her to check on time before taking the meds to make sure it's 8 hours in between. Client understoods and had her told me the time I told her. Also wrote the times (10 pm and 6 am) and pasted it on her wall just to make sure. Left almost 3:30 pm already.
Jan. 03 (Friday) = Dog outside. Client awake. Back pain. Says she was about to put off x-ray today as she felt better last night after taking the pain med. 10 pm. But pain came back this morning. Massage back with pain cream and used warm compress. Made client coffee and a boiled egg while shampooing vacant room. Then back on warm compress at the back, then pain cream, says now too hot, and back on compress again with mild water. Son called on my cp, asking for x-ray, told him I'm waiting for call but haven't gotten a call yet. Says he will call them. Got a call from her son again and says mobile physician says client declined it, but ot that we know off, so they'd check the x-ray dept. and they'd give me/us a call. Client took Hydrochlorothiazide passed 10 am. Checked mail and got her check. Client also wrote three checks: to cashed in $40 and one check to pay me for $8.40 (El Pollo Loco) and $2+ (Rite-Aid yesterday for another Bride Mix bag,) total of $10.40. third check $350 for agency. Client asked for hr balance and reminded her she was behind for 3 weeks. But says will pay for only this week and will put off the 2 weeks until next week or next check. Reminded her the Pest Control bill again and $500 for Chase but says won't write check for Chase again. Helped client changed dress and she put lipstick on. Talked to her about going to the vacant room just in case tech guy comes I'm not back yet, was waiting for their call but hadn't received a call yet and it's passed 12 noon, so decided to run errands instead since they'd call first. Left 11:40 'sh am. Deposited check on Citibank, Julian: teller. Cashed in $10.40 and the $40 for her. Went to Popeyes next and got mild, leg and thigh chicken and also regular cole slaw. Paid with client's check. At Vons next and got the dog food, canned beans, strawberry shortcake, liver patte. Paid with client's cash as well. Back almost 12:30 pm. Returned change to client with deposit receipt, $20+. Preparing lunch when tech guy arrived. asked him to wait for a few minutes as I'm going to get client ready as we hadn't receive call his on his way. Client walked into living room. Had her chest x-rayed and client walked back to vacant room for her t-back x-ray. Client had a hard time lying flat on her back with cassette so back x-ray hadn't done. Planed for a re-schedule. Called client's son and told him and he talked with the tech guy. Had warmed up client's lunch and served. Client had Popeyes leg and thigh, with some cole slaw, and the brisket and water for drink then called her son again. Told him client having lunch and talked about the x-ray again. Says they'd see what to do, they'd be here tomorrow. Client had Ibufrofen pain med. at 2 pm and had put about 5 tablets again on bedside table and told her time of taking it: same time like she did, one at 10 pm after eating something and after 8 hours, at 6 am. Reminded her to check on time before taking. Pointed out on the note about the time I pasted on the wall so she would remember. Reminded client dinner, told her sandwich that was prepared for her and baked potato and corn on fridge. Client made payment for Pest Control and also made a note for them to go with it. Jan. 04 (Saturday) = Client already awake, still in pain. Massaged back with cream and warm compress as well. Tidied up kitchen and made tea and scrambled egg. Had tea and scrambled egg. Did laundry and dryer. Turned tv on. Changed bedsheets, used new one (purple) pillow cases but not he comforter (himulmol) needs washing. Client fall asleep after bedsheet changed. Emptied commode and rinsed,swept bedroom, chaged towel rugs. Wiped kitchen floor, table, counters. Client awakened 11:20 'sh am. Says she had called CVS and told of the night cream or $8. asked CVS pharmacy to be called and asked asked if cream is good thing to use on her condition. I spoke to male pharmacist as client passed the phone to me, says yes, topicla cream can be use in addition to client's prescribed med. Gave me $20 for Popeyes. Left almost 12 noon. Got leg and thigh chicken with fries and cole slaw. @ CVS next and got 2 pain cream(+ 1 free) Diet Coke, but not enough $ for night cream. On the way back and got call from client's granddaughter saying she just spoke to her grandma and also was told I am on grocery. And she passed phone to her dad, who asked me If I think client needs hospital bed and says they're on their way now. Woke up client for lunch, the chicken thigh, cole slaw, biscuit, and Diet Coke for drink. Client wanted to take pain med. before lunch, says it really hurts and didn't took med at 6 am, but told her she have to eat first before taking med, says ok. Client had lunch. Prepared tuna sandwich for tomorrow, rice-a-roni, beans, and steamed spinach and the chicken leg and cole slaw too. returned change $2+. Took pain med @ 1 pm and rubbed her back. Prepared bp meds for tomorrow. Around 1 pm, finishes things, 1:30 'sh pm, client wanted 1 hubby's coat and some coat and blouse for her granddaughter to be given ad I found it. Put more pain med in her bedside table. Explained the time she could take again and also wrote time down paper, pasted on wall. (9 pm, 5 pm, 1 pm) and told her to check time first and eat before med. Left already 2:11 'sh pm.

Jan. 02 (Thursday) = Dog outside. Client sleeping. But says about the pain when awake.Client had her tea and a waffle. Took Hydrochlorothiazide after, around 9:40 'sh am. Massage her back with pain cream and also used warm compress. Turned on tv and shampooed vacant room. Tidied up kitchen area. Client says a guy called, the tech., or for her x-ray tomorrow. Guy says arriving passed 1, but client says I leave at 1 pm and says the guy says he'd be here between 11 to 11:30 am instead. Laundry done. Wants to try and have shower since mobile physician is coming today. Prepared shower.
