Jan. 20 - Jan. 26

Jan. 20 (Monday) = Dog outside. Client awake. Didn't heard complain about back pain like she usually does, but complains of itching. Rubbed back and used hot water. Hadn't touched pain med. Had tea and a waffle and took Hydrochlorothiazide and antibiotic before 10 am. Sponged bath, changed clients gown and changed bed sheets too. Client fall asleep, tv turned on. Tidied up kitchen. Laundry. Texted her son about mobile physician coming but didn't know time, and told client's son about me back on my other job, 2 - 10 pm, told him to call office but says says things about Merrillee talking to him about overtime but how come I have 8 hours with other patient and 4 hours with his mom, makes it 12 hours. Told him its two different patients. Still wants me to STAY until mobile physician arrives. Client had 1 vicodin tab about 11:30. Left passed 11:35 am to get lunch @ Burger King, 2 original chicken sandwich with coupon, value fries and sundae pie. Paid $8.+ with my card. Client had lunch passed 12 noon. Had an original chicken sandwich, value fries and water for drink. Mobile physician arrived before 1 pm. Checked on client. Talked to me after that it wasn't shingles like what client had before, more of like scabies and it is contageous specially with weak immune system. But told me not to tell client and told me to let him know if I ......... Called Rite-Aid pharmacy twice t ocheck for prescription but not yet. Gave client Atenolol 1:30 pm before leaving. Doesn't want to pay office yet, says will have to wait for check. Reminded her she's already 3 weeks behind. Left passed 1:35 pm.

Jan. 21 (Tuesday) = Dog was outside. Called Discount Pest Control about rescheduling their service at client. Also called Rite-Aid pharmacy and asked if there's any ready new prescription, talked to the Asian, male pharmacist and says 4 prescription is ready to be pick up. Client awake and itches and told her I need to pick up her meds. Client had tea and had taken Hydrochlorothiazide 9:30 am. Client wrote check for Rite-Aid and asked for tangerine nail polish, soda pop, and spoke to Claudia for a cream for hand for dead skin. Picked up meds, consulted with the male, Asian pharmacist. One med wasn't covered with client's insurance and paid $25 for it, total spent $55+ with client's check: Got Diet Coke, the tangerine nail polish and hand cream, peeler. Arrived back and client had 1 muffin before med. Took green med for her itch 10 am and the Guafenesin and put cream (the three times / day cream) and assisted on changing her gown. Had changed bed sheets and the linens. Tidied up kitchen. Spoke to Merrilee about client condition, about my concern of "it." Was told to wear gloves. Laundry and dryer. Client fall asleep. Had texted her son about it. Woke up client to let her know I'm going get lunch and that I would pay for it first since she said,"where' are you going to get the money from?" Went to Popeyes and got leg and thigh and biscuit, and paid $3+ with my card. Dropped by to Rite-Aid and got gloves as I had ran out of gloves. Cost $8.99. Got back and warmed up food. Client had lunch around 12 noon. Had Atenolol taken. More cream at her back. Had shampooed vacant room carpet and changed bed sheet in there. Was washing back door stairs when received call from client's son. Had told client at 4 pm she have to take the green med. and that it is every 6 hours. Also asked her about vicodin, and says she took 2halves (1 tablet) early morning, maybe 8 or around 8 am. Left already passed 1:30 pm. Had called client 4 pm, reminded her of taking med.

Jan.22 (Wednesday) = Client already aware,  says back pain. Hadnt took any pain med lately she said. Client had tea, took hydrochlorpthiazode amd 1 vicodine.  Muscle rib at the back. Tidied up kitchen. Tidied up client's bedroom. Wants to call hospital and ask wjst to dp with her back pain, told her RN Mary of Scropps home care services is coming at 11 am. Client got upset and doesn't want visitors or anyone coming,  saus sh e jad a bad might and in pain amd jist wamted to rest, askede to call Mary amd tell her mot to come today. Called Mary and told her situation. Says shw will just come on Friday instead. Put mudcle rib pn client's back, them used cold towel as sje said its hot now. Changed client's dress, turned on tv. Decided on Subway for lunch.  Laundy continued and dryer. Wsnted gootlong hotdog and wheat bun to be returned on Smart and Final, wrote check for dog f food and pther items she wants. Left about 11 or passed 11 am. Got taffy candy, nachos, paper towel, soup. Marie Callender's tv dinner (rice and batterered fish) and dog dog food. Returned items, bums is accepted, credit given but footlomg hotdog wasn't accepted. Paid with client's check. At Subway next and got tuna sandwich.  Paid $5.40 with my card.  Back around 11:30 am. Warmed up lunch. Client had half of the tuna sandwich and water for drink. Fall asleep. Had Atenolol a little after 1 pm. Hot towel on back, and then the itch cream (the 3x/day cream,) explained meds (pain med: vicodin) and green itch med hydroxyzine to client) wrote note on what each meds is for and how often to take it. Also told her both the am and pm meds in the kitchen and the meals prepared.  Checked phone is working properly and told client home care's office is pasted on wall and she could call case manager anytime if she needs help someone to be there. Left 1:35 'sh pm.

Texted client's son around 4 pm (?) on client's condition, and been doing it the passed weeks. Son called me and spoke on client's condition, about Scripps, Sandy, and what she said about his mom's condition, and him concerned about client's being alone tomorrow since I'm off and asked me if I'd be able to come. I said I already have plans and says she would have to call Merrilee for it.

Jan 23 (Thursday) = OFF!

Jan. 24 (Friday) = Client awake, lying in bed. Says she's in a bad condition: back hurts and awake most time of the night. Meds. bowls were empty on the bed. Had 1 vicodin and 1 Ibufrofin around 9:15 'sh am. Doesn't want anything to eat for the meantime. Tidied up bedroom area. Washed dishes, tidied up kitchen area. Continued laundry and dryer. Got a voicemail from Mary, Scripps home care RN, saying she'll be there between 12 - 12:30 pm. returned call. Says she'll be coming on Monday instead as she have patients that called, she needs to see. Swept client's bedroom area, emptied commode. Made client a waffle, which she ate. Had the hydroxyzine (green tablet) about 10 am. Put itch cream on back. Back rubbed with hair brush. Changed dress. Changed bed sheet. Got call from client's son,asking updates on client, if RN, Mary's coming, told him RN will be coming on Friday, clients condition, him talking to Merrilee about adding more hours/shifts if needed, learned client behind her payment for office for 1 month now, and says he called his mom last night and was told by client I was there/worked yesterday so he thought I went to work yesterday, asked if client was confused. Decided on having the hot dog or lunch. Folded dry clothes and bedsheets. Client fall asleep. Had prepared lunch. Client had footlong hotdog sandwich on Hallah bread, steamed spinach and water for drink. Prepared clients pain meds (5 tablets vicodin cut in halves) and hydroxyzine, 5 tablets. Fall asleep after lunch. Showed client the mails: 3 from Scripps, and 1 bill from blood draw from mobile physician, $213. Since I'm going to the office, asked client if she wanted to pay for the office as she was 1 month behind. Says she's been thinking of it but doesn't want to get money from her savings, asked me the date today and says she have to wait for her check. Says she will take care of it on Monday, hopefully. Client took Guafenesin 1 pm, keeps on dropping atenolol on the bed. Hadn't taken it. Explained her the vicodin and hydroxyzine meds., time she could take it, every 6 hours as needed. Let her know there's soups and meal for her meal. Left passed 1 pm.

Jan. 25 (Saturday) = Client awake. Back pain. Had tea and fried egg. Took hydrochlorothiazide and Guafenesin about 9:30 am. Changed bed sheet. Continued laundry and dryer. Put itch cream on back. Tidied up kitchen. Paid me $14 check for what she owes me for the lunches (Subway, Burger King, Popeyes) for the past days. Then wrote another check for Smart & Final. Had Ibufrofin and half of vicodin 11 am for pain. Left passed 11 am. Got taffy, dog food, paper towel, Diet Coke 1 tv dinner and called client for the lunch meat, decided on corned beef. Also got trail mix and dates with nuts. Paid by check. Had checked items and realized I forgot the corned beef so went back inside to get it. Spent $6+ with my card. Back passed 12 pm. Prepared lunch. Client had corned beef sandwich on Hallah bread, steamed spinach and Diet Coke for drink. Says corned beef wasn't as good as Akins. Prepared meals for later and tomorrow. Prepared steak and sweet corn, sliced grilled hot dog and baked potato, corned beef sandwich, Had Atenolol passed 1 pm. Prepared client's meds., and explained time of taking it, every 6 hours when needed. Meds on bedside table and told her home care's phone number was pasted on her wall if she needs someone anytime. Left almost 1:30 pm.

Jan. 26 (Sunday) = OFF!
