Feb. 20 (Thursday) = OFF! Received voicemail from mobile physician's office about health care provider dropping by today for a visit. Told staff I'm off today but works tomorrow, Friday and Saturday, 9 am - 1 pm.

Feb 10 - Feb 17

Feb. 10 (Monday) = Client lying bed. Told me I should have called her first before coming in as she doesn't have money to pay the agency. Was worried about it. Put dog outside, tidied up bedroom (dry, dog poop on of walker) and made client tea and 2 mini pancakes. Hadn't complained of pain. Had tea and ate half of 1 pancake, then requested for fried egg. Served 1 sunny side up. Took her am med. Hydrochlorothiazide and asked if she could take pain med as she is in pain. Says hadn't taken any pain med since yesterday as she thinks its making her sick, feeling sick yesterday.  Gave her 1 Ibufrofin and 1/2 Vicodin. Meds taken 9:45 am. Client was looking for her SS id. Says she had spoken to a female the other day and was told her SS number is written in her card, but she doesn't have the card. Client lied down in bed. Filled in dog's water bowl (empty when I came in.) Emptied commode, changed towel rugs and soaked. Tidied up kitchen, washed dishes. Client fall asleep. Swept bedroom. Folded dry towels. Client was looking for coins/money for lunch, found .85 cents. Dialed bank for  her as she wanted to check her balance; teller Amber. Called her bank again and she ordered checkbook: employee, Jose. Decided Vons for lunch. Wrote check for it. Had tv turned on. Left 11:10'sh am. Got lox, Diet Pepsi, white cake small slice, dog foods, canned sweet corn, tomatoes, zucchini. Paid with check, $22.13. Back, client still watching tv. Prepared lunch. Served lox sandwich, slices of tomatoes, steamed broccoli and zucchini, cream cheese and Diet Pepsi for drink passed 12 pm. Client ate well, but didn't touched small slice of cake when served. Says she was already full. Gave Atenolol 12:35 pm. Saw the mailed bills returned back, .21 cents more stamps due. Opened water bill for client, she decided to pay for it, $96.39. Wrote $350 for office before 1 pm. Asked me her balance, told her $1741 more. Dropped off water bill payment at post office drive-thru box o my way out.

Feb. 11 (Tuesday) = Client lying in bed, was cold, turned heater higher.  Doesn't want any drink. Had one, lightly noboiled egg. Had given am med., 9: 40 'sh am. Laundry continued.  Tidied up bedroom and kitchen area. Client said she had remaining lox and the kaiser roll last night. And decided for the ready chicken stew for lunch. Merrilee and boss stopped by, spoke to client and just said hi. Client watching tv. Called SSS and had requested replacement (SS5 form) for client, says will be received in within 2 weeks. Prepared lunch and made Uncle Ben's rice and sauteed some spinach for dinner. Warmed up chicken stew. Served lunch a little passed 12 pm, served chicken stew and sweet corn, water for drink. Client ate lunch well. Gave her Atenolol 12:46 pm. Brought in Sunday's newspaper. (Left 2 Ibufrofin tablet) i, in a small container.

NOTE: No complains of pain heard throughout shift. Client says about hearing a man talking twice, and she thought neighbors or dreaming.

Feb. 03 - Feb. 09

Feb. 03 (Monday) = Client awake, lying in bed. Didn't heard complain about back pain. Tidied up room, bowls and some papers on the bed. Says she was feeling bad last night, hadn't really had sleep. Made 2 mini pancakes and tea which client had before 9:30 am. And client had her hydrochlorothiazide. Told client her SSS check arrived on the mail. Spoke to me about her balance with the office. Warm towel on her back wen she started complaining a bit about the back pain. Then changed dress, and bedsheets, pillow cases. Swept bedroom and emptied commode, sanitized and had changed towel rugs. Continued laundry and dryer. Client had Ibufrofin and half of vicodin before 11 am. Told her I'm ready to deposit her check, so she spoke to me again about her balance with office. Says she doesn't have enough to pay balance fuul, asked me to dial her son's number that maybe he could lend her money. Client spoke to her son, and passed phone with me. Talked to client's son, about his mom's balance with office and told him her bills as well. Client does't want to make check for the $500 for Chase bank for her bills but wrote $25 to cash in for lunch. Texted her son about having hard time with client with the amount for Chase bank, and client not making the check later. Also doesn't want me to pick up Rite-Aid meds refill today. Left passed 11:30 am, deposited client's check on Citibank, teller Araceli and cashed in $25 for lunch. At Burger King after and got Alaskan fish sandwich and original chicken sandwich (2 for $5), and value onion ring. Paid with client's money. Back a little passed 12 noon. Client had lunch, the Alaskan fish sandwich, onion rings and water for drink. Returned change, $18. Folded dry laundry. Complains of itch. Client took her one last hydroxyzine. Also took Atenolol before 1 pm. Wrote check to pay half of the balance ($2975),client had written check for $1741. Says she will write a letter for office tomorrow to explain her situation. Left 3 halves of vicodin on bedside table, explained client she can take next pain med if she needs it by 5 pm. Turned on tv.Left 1:11 pm.

Feb. 04 (Tuesday) = Client awake lying in bed. Hadnt heard complain of pain or anything but is looking for her SS card. Laundry continued. Client had tea and ate 1 pancake of two. Shampooed vacant room. Client had hydrochlorothiazide med. Body cream on clients back. Chamged her clothes and changed bedsheet. Swept bedroom area. Tidied up kitchen. Reminded client I have to pick up her refill on Rite-Aid, doesn't want to at first as she worries about money.  Doesnt want to pay for her bills too when I talked to her about it. Also thinls on how she would pay the remaining balance of her from the office. Had spoke to her about calling her bank to order her checkbook, refused. Also spoke to her anout checking her balance, teller was female, client said she jave over $2200+. Then wrote check for Rite-Aid. Left passed 11 am. Bought phone baby oil and phone cord and picked up refills (Atenolol and Hydroxyzine
) Back passed 11:45'sh am. Client still watching tv, says shes wasnt hungry. Had prepared lunch.  Made cormed beef sandwich, and steamed spinach and baked potato. Client had half of sandwich and half baked potato and water for drink.  Client ate but shared sandwich with dog. Client complains back pain passed 12:30 pm. Took one ibufrofin and half vicodin, 1 pm. Had given me $9 for tomorrow on 99 Cents. Left 1:20'sh pm.
